翼状胬肉切除术联合自体角膜缘干细胞移植术对翼状胬肉患者的治疗效果

摘　　要：目的研究翼状胬肉应用翼状胬肉切除术与自体角膜缘干细胞移植术联合治疗的效果。方法选取2020年1月—2021年2月该院收治的248例翼状胬肉患者为研究对象,按随机数表法分为两组,各124例。对照组行翼状胬肉切除术,观察组行翼状胬肉切除术联合自体角膜缘干细胞移植术,对比两组治疗效果、围术期指标、并发症及术前术后裸眼视力、角膜散光度。结果治疗总有效率比较,观察组(97.58%)高于对照组(84.68%),差异有统计学意义(χ^(2)=12.769,P<0.05);与对照组比较,观察组手术时间更长,角膜上皮愈合时间、结膜瓣愈合时间及住院时间均更短,差异有统计学意义(t=17.544、10.467、12.198、8.379,P<0.05);与对照组比较,观察组术后裸眼视力更高,角膜散光度更低,差异有统计学意义(t=14.367、8.622,P<0.05);并发症发生率比较,观察组(2.42%)低于对照组(11.29%),差异有统计学意义(χ^(2)=7.641,P<0.05)。结论翼状胬肉患者应用翼状胬肉切除术与自体角膜缘干细胞移植术联合治疗,可明显提高疗效,有利于缩短患者恢复时间,减少并发症,并提高患者裸眼视力,改善患者角膜散光度。Objective To study the effect of combined treatment of pterygium with pterygium excision and autologous limbal stem cell transplantation.Methods A total of 248 patients with pterygium who were treated in the hospital from January 2020 to February 2021 were selected as the research objects,and they were divided into groups according to the random number table method,with 124 cases in each group.The control group underwent pterygium excision,while the observation group underwent pterygium excision combined with autologous limbal stem cell transplantation.The treatment effects,perioperative indicators,complications,and uncorrected visual acuity and corneal astigmatism before and after surgery were compared between the two groups.Results Comparing the total effective rate of treatment,the observation group(97.58%)was higher than the control group(84.68%),and the difference was statistically significant(χ^(2)=12.769,P<0.05);compared with the control group,the observation group had longer operation time,corneal epithelium healing time,conjunctival flap healing time and hospital stay were shorter,and the differences were statistically significant(t=17.544,10.467,12.198,8.379,P<0.05);compared with the control group,the observation group had higher uncorrected visual acuity,corneal astigmatism was lower,and the difference was statistically significant(t=14.367,8.622,P<0.05);the incidence of complications in the observation group(2.42%)was lower than that in the control group(11.29%),and the difference was statistically significant(χ^(2)=7.641,P<0.05).Conclusion The combined treatment of pterygium excision and autologous limbal stem cell transplantation in patients with pterygium can significantly improve the curative effect,shorten the recovery time of patients and reduce complications,and improve uncorrected visual acuity and corneal astigmatism of patients.
