内镜套扎联合生长抑素对肝硬化食管静脉曲张出血的疗效

摘　　要：目的分析肝硬化食管静脉曲张出血联合应用内镜套扎与生长抑素治疗的效果。方法选取该院2017年1月—2021年1月收治的50例肝硬化食管静脉曲张出血患者,以随机数表法分为两组,对照组25例予以生长抑素治疗,研究组25例加用内镜套扎术治疗,比较两组治疗效果。结果研究组止血时间和住院时间均明显较对照组短,研究组输血量少于对照组,住院费用高于对照组,差异有统计学意义(t=6.657、7.729、3.729、2.910,P<0.05)。研究组治疗总有效率为96.00%,对照组为68.00%,组间对比差异有统计学意义(χ^(2)=4.878,P<0.05)。研究组治疗后HVPG、RBC水平低于对照组,Hb水平高于对照组,差异有统计学意义(P<0.05)。研究组不良反应发生率为16.00%,与对照组的12.00%比较,差异无统计学意义(P>0.05)。两组治疗1个月后生活质量评分比较,差异有统计学意义(P<0.05)。结论内镜套扎与生长抑素联合应用,能够显著缩短患者止血时间,减少患者输血量,改善患者HVPG,具有显著临床疗效,且安全性高,对患者预后改善具有重要意义。Objective To analyze the effect of combined endoscopic band ligation and somatostatin in the treatment of esophageal variceal bleeding in liver cirrhosis.Methods A total of 50 patients with liver cirrhosis and esophageal variceal hemorrhage who were treated in the hospital from January 2017 to January 2021 were selected and randomly divided into two groups.25 cases in the control group were treated with somatostatin,and 25 cases in the study group were added the endoscopic band ligation for treatment.The therapeutic effects of the two groups were compared.Results Hemostasis time and hospital stay in the study group were significantly shorter than those in the control group,the blood transfusion volume in the study group was less than that in the control group,and the hospitalization costs were higher than those in the control group,and the difference was statistically significant(t=6.657,7.729,3.729,2.910,P<0.05).The total effective rate was 96.00%in the study group and 68.00%in the control group,the difference between groups was statistically significant(χ^(2)=4.878,P<0.05).After treatment,the levels of HVPG and RBC in the study group were lower than those in the control group,and the level of Hb was higher than that in the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the study group was 16.00%,compared with 12.00%in the control group,and the difference was not statistically significant(P>0.05).Comparison of quality of life scores between the two groups after 1 month of treatment,and the difference was statistically significant(P<0.05).Conclusion The combined application of endoscopic band ligation and somatostatin can significantly shorten the hemostasis time,reduce the blood transfusion volume,and improve the HVPG of patients.
