关爱式教育与随访管理对促进青少年人工流产后落实长效可逆避孕方法的效果分析

摘　　要：目的:探讨关爱式教育与随访管理对促进青少年落实长效可逆避孕方法(LARC)的效果.方法:选取因意外妊娠行人工流产的青少年女性共264例,采用随机数字表法分为观察组和对照组,各132例.对照组采用常规流产后关爱服务,观察组采用关爱式教育与随访管理.统计分析两组青少年LARC知识问卷得分,术后即时LARC落实率、术后6个月LARC续用率、重复流产率、终止率及避孕效果满意率等方面的差异.结果:干预前两组问卷回答正确率在50.0%以上的条目(均为3个)及LARC知识得分无统计学差异(P>0.05);干预后观察组回答正确率在50.0%以上的条目及LARC知识得分(10个,10.0±2.0)均好于对照组(5个,6.3±1.8)(P<0.05),且观察组干预前后比较也有差异(P<0.05).观察组术后即时LARC落实率(51.5%)、术后6个月续用率(48.5%)、终止率(2.9%)、避孕效果满意率(97.1%)均好于对照组(13.6%、9.1%、72.2%,P<0.05).两组重复流产率(0.8%,2.3%)无差异(P>0.05).结论:关爱式教育与随访管理有助于提高青少年人工流产后LARC的认知及即时落实率,提高后续使用率,减少终止率,提高避孕效果满意率.Objective:To explore the effect of caring education and follow-up management for improving the long-acting reversible contraceptive(LARC)used of female adolescents after abortion.Methods:A total of 264 female adolescents who wanted abortion due to unexpected pregnancy were selected and divided into study group and control group by ran-dom number table method(132 cases in each group).The female adolescents in the control group were given conven-tional PAC services,and the female adolescents in the observation group were given caring education and follow-up management.The questionnaire scores of LARC knowledge of the female adolescents in the two groups were analyzed.The differences of the rate of LARC used after abortion immediately,the continuation rate of LARC within 6 months after abortion,the rate of repeated abortions,the rate of discontinuation rate,and the satisfaction of contraceptive effect of the female adolescents in the two groups were also analyzed.Results:Before intervention,there were no statis-tically significant differences in the number of items with correct answer rate above 50.0%(3 items)and the LARC knowledge score of the female adolescents between the two groups(P>0.05).The number of items with correct an-swer rate above 50.0%(10 items)and the knowledge score of intrauterine device(10.0±2.0)of the female adolescents in the observation group after intervention were significantly better than those(5 items and 6.3±1.8)of the female ad-olescents in the control group(P<0.05),and which of the female adolescents in the observation group after interven-tion had significantly different from those before intervention(P<0.05).The rate of LARC used after abortion imme-diately(51.5%),the continuation rate(48.5%),the discontinuation rate(2.9%),and the satisfaction of contraceptive effect(97.1%)of the female adolescents in the observation group after intervention were significantly better than those(13.6%,9.1%,and 72.2%)of the female adolescents in the control group(P<0.05).There was no significant differ
