摘 要:目的探讨盐酸哌甲酯缓释片联合脑电生物反馈加感统训练治疗儿童注意缺陷多动障碍(ADHD)的临床疗效及可行性。方法2018年12月至2020年12月三门峡中心医院收治的ADHD患儿128例,根据治疗方案不同分为对照组67例(脑电生物反馈加感统训练治疗)、观察组61例(脑电生物反馈加感统训练+口服盐酸哌甲酯缓释片治疗)。6周为1个疗程,持续2个疗程。比较两组治疗前后SNAP-IV评定量表评分、智力情况、脑电波频率。结果治疗后两组SNAP-IV评定量表评分(注意力不足、过动/冲动、对立违抗、全量表)均明显下降,C-WISC量表评分(全量表智商、言语量表智商及操作量表智商)均明显升高,且观察组较对照组改变更明显,差异均有统计学意义(P<0.05)。治疗后两组β波、SMR波明显升高,θ波明显下降(P<0.05),且观察组β波、SMR波升高幅度大于对照组,θ波下降幅度大于对照组(P<0.05)。两组均无严重不良反应发生。结论脑电生物反馈加感统训练治疗联合盐酸哌甲酯缓释片治疗ADHD患儿能提高患儿行为能力、认知功能,改善脑电波频率,是一种安全、可行的方案。Objective To explore the clinical efficacy and feasibility of methylphenidate HCl extended-release tablets combined with EEG biofeedback and sensory integrationtraining therapy in the treatment of children with attention deficit hyperactivity disorder(ADHD).Methods One hundred and twenty-eight children with ADHD from December 2018 to December 2020 were selected.The sick childrenwere divided into a control group treated with EEG biofeedback and sensory integration trainingtherapy(n=67)and an observation group treated with EEG biofeedback and sensory integrationtraining therapy+oral methylphenidate HCl extended-release tablets(n=61).The treatment course was 6 weeks and 2 courses were performed.The scores of the Swanson Nolan and Pelham version IV(SNAP-IV)rating scale,intelligence,and brainwave frequency before and after treatment were compared between the two groups.Results After treatment,the scores of the SNAP-IV rating scale(insufficient attention,hyperactivity/impulsiveness,oppositional defiance and full scale)of the two groups were significantly decreased(P<0.05),while the scores of Chinese Wechsler Young Children scale of Intelligence(C-WYCSI),including full scale IQ,verbal scale IQ,and operational scale IQ,were significantly increased(P<0.05),and the change in the observation group was more obvious than that in the control group(P<0.05).After treatment,theβwave and sensorimotor rhythm(SMR)wave of the two groups were significantly increased,and theθwave was significantly decreased(P<0.05)and the increase ofβand SMR waves and the decrease ofθwave were greater in the observation group than those of the control group(P<0.05).No serious adverse reactions occurred in both groups.Conclusion Methylphenidate HCl extended-release tablets combined with EEG biofeedback and sensory integrationtraining therapy in the treatment of children with ADHD can improve children′s behavioral ability,and cognitive function,and improve the frequency of brain wave.It is a safe and feasible solution.
