超声乳化白内障吸除术在闭角型青光眼治疗中的临床效果分析

摘　　要：目的 探讨对闭角型青光眼患者采用超声乳化白内障吸除术完成治疗后获得的临床效果。方法 选取2008年2月—2021年10月收治的60例闭角型青光眼患者进行治疗研究,随机分为常规组(采用小梁切除术完成疾病治疗)和研究组(采用超声乳化白内障吸除术完成疾病治疗),各30例。比较两组患者治疗总有效率、房角粘连闭合角度、眼压、视力水平以及并发症(虹膜萎缩、角膜水肿、瞳孔散大、视网膜脱落)总发生率。结果 研究组治疗总有效率(93.33%)高于常规组(66.67%),差异有统计学意义(χ^(2)=6.666,P<0.05);研究组房角粘连闭合角度(58.56±12.25)°小于常规组房角粘连闭合角度(206.21±38.39)°,差异有统计学意义(t=20.068,P<0.05);研究组眼压(11.35±2.59)mmHg小于常规组眼压(16.52±2.63)mmHg,视力(0.78±0.13)高于常规组视力(0.59±0.07),差异有统计学意义(t=7.671、7.048,P<0.05);研究组并发症(虹膜萎缩、角膜水肿、瞳孔散大、视网膜脱落)总发生率(3.33%)低于常规组(46.67%),差异有统计学意义(χ^(2)=15.022,P<0.05)。结论 超声乳化白内障吸除术的有效应用,可显著增强疾病治疗效果,显著改善房角粘连闭合角度、眼压、视力水平,并减少虹膜萎缩、角膜水肿、瞳孔散大、视网膜脱落系列并发症发生,促进闭角型青光眼有效治疗以及预后水平提升。Objective To investigate the clinical effect of phacoemulsification and cataractectomy in patients with angle-closure glaucoma. Methods A total of 60 patients with angle-closure glaucoma who were treated from February 2008 to October 2021were selected for treatment research, they were randomly divided into a routine group(completing disease treatment by tra-beculectomy) and a study group(using phacoemulsification to remove cataracts), with 30 patients in each group. The total effective rate of treatment, angle of angle adhesion closure, intraocular pressure, visual acuity, and the total incidence of compli-cations(iris atrophy, corneal edema, mydriasis, retinal detachment) were compared between the two groups. Results The total effective rate of the study group(93.33%) was higher than that of the routine group(66.67%), the difference was statistically significant(χ^(2)=6.666,P<0.05). The angle of adhesion closure in the study group was(58.56 ±12.25)° smaller than that in the routine group(206.21±38.39)°, the difference was statistically significant(t=20.068,P<0.05). The intraocular pressure(11.35 ±2.59) mmHg of the study group was lower than that of the routine group(16.52±2.63) mmHg, and the visual acuity(0.78±0.13)of the study group was higher than that of the routine group(0.59 ±0.07), the difference was statistically significant(t=7.671,7.048,P<0.05). The total incidence of complications(iris atrophy, corneal edema, mydriasis, retinal detachment) in the study group(3.33%) was lower than that in the routine group(46.67%), the difference was statistically significant(χ^(2)=15.022,P<0.05).Conclusion The effective application of phacoemulsification cataractectomy can significantly enhance the treatment effect of the disease, and significantly improve the angle of angle adhesion and closure, intraocular pressure, and visual acuity.
