摘  要:反复暴露在爆炸冲击波下是导致军事人员健康不良的主要原因,特别是爆炸冲击所致的脑外伤、神经症状及长期认知障碍,极大地降低了患者的生存质量。目前,确定爆炸的损伤效应仍主要依赖于脑震荡的症状报告和与手术减量相关的神经认知评估,临床中急需通过一些生物指标进一步诊断和监测爆炸冲击所致的神经系统损伤。已证明神经丝轻链(NFL)是一种与炎症、神经退行性疾病、创伤和脑血管疾病等多种神经疾病密切相关的蛋白,其在脑脊液和血液中的水平与神经元损伤的程度成正比。该文从NFL在神经系统中发挥的作用及其与创伤性脑损伤(TBI)、阿尔茨海默病(AD)、帕金森病(PD)、肌萎缩侧索硬化症(ALS)存在的相关性等方面,就NFL在爆炸冲击所致神经系统疾病中的生物标志性作用进行综述。Repeated exposure to explosive shock waves is the main cause of poor health of military personnel,especially brain trauma,neurological symptoms and long-term cognitive impairment caused by explosive shocks,which greatly lowers the quality of life of patients. The current determination of the damage effect of explosion still depends on symptom reports of the concussion and neurocognitive evaluation related to the surgical reduction. There is an urgent need for some clinical indicators to help diagnose and monitor the nervous system damage caused by the explosion impact. It has been proved that the neurofilament light(NFL)chain is a protein closely related to inflammation,neurodegenerative diseases,trauma and cerebrovascular diseases and other neurological diseases. This article reviews the role of NFL as a biomarker in the nervous system and its correlation with traumatic brain injuries(TBI),Alzheimer′s disease(AD),Parkinson′s disease(PD),and amyotrophic lateral sclerosis(ALS).
