摘　　要：目的系统评价肝脏硬度是否可预测肝细胞癌患者的预后。方法检索PubMed、Web of Science、中国知网和万方数据库中有关肝脏硬度与肝细胞癌患者预后的文献,并按照纳入和排除标准进行筛选,利用RevMan 5.3软件进行统计分析。结果共纳入文献18篇,包含肝细胞癌患者3152例。meta分析结果显示:高肝脏硬度值的肝细胞癌患者治疗后的无复发生存时间更短[HR=1.09,95%CI(1.04,1.14),P<0.0001],5年无复发生存率更低[OR=2.67,95%CI(2.08,3.44),P<0.00001],5年复发率更高[OR=0.19,95%CI(0.09,0.38),P<0.00001],总体生存时间更短[HR=2.42,95%CI(1.45,4.03),P=0.0007],5年总体生存率更低[OR=2.89,95%CI(2.18,3.83),P<0.00001]。结论检测肝脏硬度可预测肝细胞癌患者的预后。Objective To systematically evaluate the association between liver stiffness and the prognosis of patients with hepatocellular carcinoma(HCC).Methods We searched PubMed,Web of Science,Wanfang,and CNKI databases for relevant literatures about the association between liver stiffness and the prognosis of HCC patients.Then we selected eligible literatures according to the including and excluding criteria,and we used Revman 5.3 software to perform statistical analysis.Results A total of 18 articles with 3152 HCC patients were included.Meta results showed that HCC patients with higher liver stiffness had shorter recurrence free survival time[HR=1.09,95%CI(1.04,1.14),P<0.0001],lower 5-year recurrence free survival rate[OR=2.67,95%CI(2.08,3.44),P<0.00001],higher 5-year recurrence rate[OR=0.19,95%CI(0.09,0.38),P<0.00001],shorter overall survival time[HR=2.42,95%CI(1.45,4.03),P=0.0007],and lower 5-year overall survival rate[OR=2.89,95%CI(2.18,3.83),P<0.00001].Conclusion Liver stiffness measurement can predict the prognosis of HCC patients.
