脾脏错构瘤一例病理学分型及影像学表现并文献复习

摘　　要：目的探讨脾脏错构瘤不同病理类型的影像学特点。方法对1例经手术后病理证实的脾脏错构瘤的影像学及病理学特点进行分析并结合文献复习。结果文献报道有影像学及病理学资料的脾脏错构瘤共125例,按组织病理学分为红髓型(109例)、白髓型(2例)、纤维型(6例)和混合型(8例),本例为红髓型。脾脏错构瘤CT平扫呈等或稍低密度,脂肪及钙化少见;MR平扫T1WI呈等或稍低信号,T2WI纤维型呈低信号、其余类型呈高或稍高信号;动态增强扫描红髓型及混合型动脉期呈不均匀轻度强化或明显强化、门静脉期及延迟期持续强化,白髓型呈轻度强化,纤维型呈延迟性强化。结论不同病理类型的脾脏错构瘤影像学表现具有一定特征性,尤其是动态增强扫描对该病的诊断和鉴别诊断具有较大价值。Objective To understand the imaging features of different pathological types of splenic hamartoma.Methods The imaging and pathological features of 1 case of splenic hamartoma after operation were analyzed and related literature was studied.Results A total of 125 cases of splenic hamartoma with imaging and pathological data were reported in the literature.According to pathology,it was classified as red pulp type(109 cases),white pulp type(2 cases),fibrous type(6 cases)and mixed type(8 cases),and this case was red pulp type.In non enhanced CT,splenic hamartoma showed equal or slightly low density,and fat and calcification were rare.In non enhanced MR,T1WI showed equal or slightly low signal,fiber type showed low signal and the other showed high or slightly high signal on T2WI.In dynamic contrast-enhanced scan,the red pulp type and mixed type showed uneven mild or obvious enhancement in arterial phase,persistent enhancement in portal vein phase and delayed phase,while the white pulp type showed mild enhancement and the fiber type showed delayed enhancement.Conclusion the imaging features of splenic hamartoma with different pathological types are characteristic,especially the dynamic enhanced scanning is of great value in the diagnosis and differential diagnosis of the disease.
