不同下斜肌减弱术联合水平肌加强减弱术治疗伴下斜肌亢进斜视的临床效果

摘　　要：目的分析下斜肌后徙术与下斜肌部分切除术分别联合水平肌加强减弱术治疗伴下斜肌亢进斜视的临床效果。方法收集2017年1月—2021年10月收治的112例伴下斜肌亢进斜视的临床资料,按不同手术方法分为后徙组58例与部分切除组54例,后徙组给予下斜肌后徙术联合水平肌加强减弱术,部分切除组给予下斜肌部分切除术联合水平肌加强减弱术。比较两组临床疗效、垂直斜视度数、下斜肌功能及视功能恢复情况。结果后徙组总有效率高于部分切除组(P<0.05)。术后,后徙组垂直斜视度数低于部分切除组(P<0.05)。后徙组下斜肌功能正常占比高于部分切除组,下斜肌功能亢进占比低于部分切除组(P<0.05)。术后,后徙组视功能改善程度优于部分切除组(P<0.01)。两组并发症发生率比较差异无统计学意义(P>0.05)。结论下斜肌后徙术联合水平肌加强减弱术治疗伴下斜肌亢进斜视的临床效果确切,可降低垂直斜视度数,促进下斜肌功能和视功能恢复。Objective To analyze the clinical effect of inferior oblique muscle recession and partial inferior oblique muscle resection combined with horizontal muscle strengthening and weakening in the treatment of strabismus with inferior oblique muscle hypertrophy.Methods The clinical data of 112 patients of strabismus with inferior oblique muscle hypertrophy admitted from January 2017 to October 2021 was collected,and according to different surgical methods,they were divided into recession group(n=58)and the partial resection group(n=54).The recession group was given inferior oblique posterior recession combined with horizontal muscle strengthening and weakening,and the partial resection group was given partial resection of the inferior oblique muscle combined with horizontal muscle strengthening and weakening.The clinical effects,vertical strabismus degree,inferior oblique muscle function and visual function recovery of the two groups were compared.Results The total effective rate of the recession group was higher than that of the partial resection group(P<0.05).After operation,the vertical strabismus degree was lower in the recession group than in the partial resection group(P<0.05).The proportion of normal inferior oblique muscles function in the recession group was higher than that of the partial resection group,and the proportion of inferior oblique hyperfunction was lower than that of the partial resection group(P<0.05).After operation,the improvement in visual function in the recession group was better than that of the partial resection group(P<0.01).There was no difference in the incidence of complications between the two groups(P>0.05).Conclusion Inferior oblique muscle recession combined with horizontal muscle strengthening and weakening is effective in the treatment of strabismus with inferior oblique muscle hypertrophy,which can reduce the degree of vertical strabismus and promote the recovery of inferior oblique muscle function and visual function.
