摘 要:目的探讨急性脑出血(ACH)部位、心电图及血清肌钙蛋白Ⅰ(c Tn-Ⅰ)的关系。方法选取2012年6月至2019年6月义乌市中医医院收治的200例ACH患者为研究对象,其中出血部位在基底节178例、小脑9例、脑叶13例。比较不同出血部位患者心电图异常情况,分析脑出血部位、心电图及c Tn-Ⅰ水平的关系。结果200例ACH患者心电图异常率为77.50%。不同出血部位患者起源异常率、复极异常率、传导异常率比较,差异均无统计学意义(均P>0.05);但总的心电图异常率比较,差异有统计学意义(P<0.05),其中基底节组心电图异常率明显高于小脑组、脑叶组(均P<0.05),而小脑组与脑叶组比较差异无统计学意义(P>0.05)。在心电图异常患者中,不同出血部位者cTn-Ⅰ水平比较,差异有统计学意义(P<0.05),其中脑叶组>基底节组>小脑组。在基底节组、小脑组、脑叶组内,心电图异常与正常患者c Tn-Ⅰ水平比较,差异均有统计学意义(均P<0.05)。Spearman秩相关分析显示,ACH患者心电图异常、cTn-Ⅰ水平与脑出血部位均呈正相关(rs=0.309、0.215,均P<0.05)。结论ACH患者的心电图及cTn-Ⅰ变化与出血部位有关,完善心电图及cTn-Ⅰ检测有助于判断病情并改善患者预后。Objective To explore the relationship between the site of acute intracerebral hemorrhage(ACH),electrocardiogram and serum troponin I(cTn-I).Methods A total of 200 ACH patients admitted to Yiwu Hospital of Traditional Chinese Medicine from June 2012 to June 2019 were selected,of which 178 patients were bleeding in the basal ganglia,9 in the cerebellum,and 13 in the lobe of the brain.The abnormal electrocardiograms of patients with different bleeding sites were compared,and the relationship among cerebral hemorrhage sites,electrocardiograms and cTn-I levels was analyzed.Results The abnormal electrocardiogram rate of 200 ACH patients was 77.50%.There was no significant difference in the abnormality rate of origin,re-grade abnormality rate and conduction abnormality rate among patients with different bleeding sites(all P>0.05).However,there was a statistically significant difference in the total abnormal rate of ECG(P<0.05).The abnormal rate of ECG in the basal ganglia group was significantly higher than that in the cerebellar group and the lobe group(all P<0.05).The difference was not statistically significant(P>0.05).Among the patients with abnormal ECG,the c Tn-I levels in different bleeding sites were significantly different(P<0.05),in which the lobe group>the basal ganglia group>the cerebellum group.In the basal ganglia group,the cerebellum group,and the lobe group,there were significant differences in the c Tn-I levels between the abnormal ECG and normal patients(all P<0.05).Spearman correlation analysis showed that abnormal electrocardiogram and c Tn-I level in ACH patients were positively correlated with the location of cerebral hemorrhage(rs=0.309,0.215,both P<0.05).Conclusion The changes of ECG and c Tn-I in patients with ACH are related to the location of bleeding.Improving the detection of ECG and c Tn-I can help to judge the condition and improve the prognosis of patients.
