完整结肠系膜切除术用于结肠癌治疗的有效性及安全性分析

摘　　要：目的 分析结肠癌应用完整结肠系膜切除术的安全性及有效性。方法 选取2018年4月—2020年9月我院接收的结肠癌患者88例,采用随机数表法将其分为对照组与研究组,每组各44例。对照组给予传统根治术治疗,研究组给予完整结肠系膜切除术。比较两组淋巴结的切除情况、围术期各临床相关指标、炎症反应及并发症。结果 研究组结肠癌Ⅰ、Ⅱ、Ⅲ期、右半结肠、左半结肠淋巴结切除数量及平均淋巴结的切除数量均高于对照组;研究组术中出血量及肛门排气、拔管、饮食恢复、住院时间、术后IL-6及CRP均低于对照组;研究组术后并发症发生率低于对照组;上述数据分别进行组间比较,差异均有统计学意义(t=3.156、7.300、7.125、6.496、6.482、6.082、14.511、10.892、10.147、4.915、14.209、49.723、8.390,χ^(2)=5.906,均P<0.05)。结论 完整结肠系膜切除术治疗结肠癌能显著缩短患者的康复进程,提升淋巴结切除率,减轻炎症反应,降低并发症的发生。Objective To analyze the safety and efficacy of complete mesocolectomy for colon cancer. Methods From Apr. 2018 to Sep.2020, 88 cases of colon cancer admitted to our hospital were divided into control group and study group by random number table method, 44 cases in each group. The patients in the control group were treated with traditional radical surgery, and in the study group were treated with complete mesocolectomy. The lymph node resection, perioperative indicators, inflammatory reaction and complications were compared between the two groups.Results In the study group,Ⅰ,Ⅱ, and Ⅲ stage of colon cancer, right and left hemicolon lymph nodes, the average number of lymph nodes resected were higher than those in the control group;The amount of intraoperative blood loss, the time of anal exhaust, extubation, diet recovery and hospitalization, IL-6 after operation and CRP levels in the study group were lower than those of the control group;The incidence of postoperative complications was lower than that in the control group;The above data were compared between groups, and the differences were statistically significant(t=3.156, 7.300, 7.125, 6.496, 6.482, 6.082, 14.511, 10.892, 10.147, 4.915, 14.209, 49.723, 8.390, χ^(2)=5.906, all P<0.05). Conclusion Complete mesocolectomy for the treatment of colon cancer can significantly shorten the recovery process of patients, improve the rate of lymph node resection, reduce inflammation, and decrease the risk of complications.
