泻南补北法治疗慢性中心性浆液性脉络膜视网膜病变的理论与应用探讨

摘　　要：慢性中心性浆液性脉络膜视网膜病变(cCSC)是一种病程超过6个月的以黄斑区浆液性神经上皮层脱离为主要特征的眼底疾病。随着社会生活节奏加快,本病的诱发因素诸如精神压力过大、过度劳累等与日俱增,使得cCSC的发病率近年来呈上升趋势。中医治疗cCSC方法众多,但缺少有关其病因病机的系统阐述。本文通过对心肾二脏生理、病理与目关系的阐述,将整体与局部辨证有机结合,从病因、病程、病位3个方面论述心肾不交为cCSC的核心病机。以此为指导,提出泻南补北的治疗法则,并根据患者病情不同提出相应方药,力求为cCSC的治疗开拓出新的理论思路及临床方案。Chronic central serous chorioretinopathy(cCSC)is a fundus disease characterized by serous neuroepithelial detachment in macular area with a course of more than 6 months.With the acceleration of the social life,the inducing factors of cCSC,such as excessive mental pressure and overwork,are increasing day by day,making the incidence of cCSC on the rise in recent years.There are many ways to treat chronic central serous chorioretinopathy in Traditional Chinese Medicine,but there is no systematic explanation about the etiology and pathogenesis of cCSC.In this paper,by elaborating the relationship of the physiology and pathology between the heartkidney and the eye,the whole and the local syndrome differentiation were coordinate combined,and the internal pathogenesis of cCSC was proved to Heart-kidney disjunction from the three aspects of etiology,disease course and disease location.As a guide,we put forward the treatment rules of Reducing the South and Reinforcing the North,and put forward corresponding prescriptions according to the different conditions of patients to develop a new theoretical ideas and clinical methods for the treatment of cCSC.
