改良小梁切除术对急性闭角型青光眼疗效及安全性分析

摘　　要：目的探讨分析改良小梁切除术对急性闭角型青光眼疗效及安全性。方法选择选取2017年12月至2019年3月收治的急性闭角型青光眼患者60例,随机将患者分为对照组(n=30)和观察组(n=30),给予观察组改良小梁切除术治疗,给予对照组常规小梁切除术治疗,比较两组术前后IOP水平,比较两组患者术后浅前房形成情况,比较两组手术后并发症发生率。结果术后7 d、180 d观察组IOP水平低于对照组(P<0.05),浅前房形成情况比较,观察组优于对照组(P<0.05),观察组并发症发生率10.00%低于对照组(26.66%)(P<0.05)。结论改良小梁切除术是将角膜缘作为基底的高位结膜瓣,在进行手术的过程中使用可调整缝线结扎,根据房水流速和滤过量来调整结扎松紧度,从而促进功能性滤过泡的形成,并维持其稳定性。改良小梁切除术可有效改善术后眼压指标,提升患者视力效果,有利于优化患者的浅前房功能,降低术后并发症的发生率。有利于帮助患者快速恢复健康。Objective To investigate the efficacy and safety of modified trabeculectomy in the treatment of acute angle closure glaucoma.Methods Sixty patients with acute angle closure glaucoma admitted from December 2017 to March 2019 were randomly divided into two groups:control group(n=30)and observation group(n=30).The patients in the observation group were treated with modified trabeculectomy,and the patients in the control group were treated with conventional trabeculectomy.The IOP levels before and after operation were compared between the two groups.The shallow anterior chamber formation after operation was compared between the two groups.The incidence of postoperative complications.Results The IOP level of the observation group was lower than that of the control group(P<0.05)at 7 days and 180 days after operation,and the formation of shallow anterior chamber in the observation group was better than that in the control group(P<0.05).The incidence of complications in the observation group(10.00%)was lower than that in the control group(26.66%)(P<0.05).Conclusion The modified trabeculectomy is to use the limbus cornea as the high conjunctival flap of the basement,at the same time,the incision of the bulbar conjunctiva and the incision of the fascial sac of the eyeball are dislocated.Before trabeculectomy,the anterior chamber is punctured,the aqueous humor is discharged,and the balance fluid is put into the incision of the anterior chamber,so as to promote the formation of functional filtering blebs and maintain their stability.Improved trabeculectomy can effectively improve the intraocular pressure index,improve the visual effect of patients,optimize the shallow room function of patients,and reduce the incidence of postoperative complications.It is helpful to help patients recover health quickly.
