电子灸联合乙哌立松治疗中风后上肢痉挛的疗效观察

摘　　要：目的:观察电子灸联合乙哌立松治疗中风后上肢痉挛患者的临床疗效。方法:选取2020年10月~2021年12月在福建中医药大学附属福州中医院脑病科及针灸康复科就诊的中风后偏瘫上肢痉挛患者60例,按随机数字表法分为对照组和观察组各30例。对照组给予单纯口服乙哌立松治疗,观察组给予电子灸联合乙哌立松治疗。分别于治疗前后采用改良Ashworth量表(MAS)和超声弹性成像杨氏模量值评定患肢的痉挛状态,采用上肢Fugl-Meyer运动量表(FMA)和改良Barthel指数(MBI)评定患肢的运动功能及日常生活活动(ADL)能力。结果:治疗后,两组患者的MAS评分、杨氏模量值较治疗前降低(P<0.05),且观察组治疗后的MAS评分、杨氏模量值低于对照组(P<0.05)。治疗后,两组患者的FMA评分、MBI评分较治疗前升高(P<0.05),且观察组治疗后的FMA评分、MBI评分高于对照组(P<0.05)。结论:电子灸联合乙哌立松治疗中风后上肢痉挛效果显著,能有效促进患者运动功能和ADL能力的恢复。Objective:To observe the clinical efficacy of electronic moxibustion combined with eperisone in the treatment of upper-limb spasm after stroke.Methods:From October 2020 to December 2021,60 patients with upper limb spasm of post-stroke hemiplegia treated in the Department of encephalopathy and acupuncture rehabilitation department of Fuzhou Hospital of TCM Affiliated to Fujian University of TCM were randomly divided into control group and observation group.The control group was treated with oral eperisone alone,and the observation group was treated with electronic moxibustion combined with eperisone.Before and after treatment,the spasticity of the affected limb was evaluated by the modified Ashworth Scale(MAS)and Young’s modulus of ultrasonic elastic imaging,and the motor function of the affected limb and activity of daily living(ADL)were evaluated by the Fugl-Meyer assessment(FMA)and modified Barthel Index(MBI).Results:After treatment,the MAS score and Young’s modulus of the two groups were lower than those before treatment(P<0.05),and the MAS score and Young’s modulus of the observation group were lower than those of the control group(P<0.05).After treatment,the FMA score and MBI score of the two groups were higher than those before treatment(P<0.05),and the FMA score and MBI score of the observation group were higher than those of the control group(P<0.05).Conclusion:electronic moxibustion combined with eperisone is effective in the treatment of upper-limb spasm after stroke,and can effectively promote the recovery of motor function and ADL ability.
