摘 要:目的分析膀胱癌患者血小板因子4(PF4)水平及水通道蛋白1(AQP1)、水通道蛋白3(AQP3)表达与疾病进展、预后的关系。方法将2016年1月—2018年1月我院92例膀胱癌作为观察组,同期体检健康者50例作为对照组。比较2组血清PF4水平及观察组癌、癌旁组织AQP1、AQP3表达情况;分析观察组临床病理特征与血清PF4及癌组织AQP1、AQP3的关系;比较不同预后患者血清PF4与癌组织AQP1、AQP3表达情况;采用Pearson相关性分析探讨血清PF4与癌组织AQP1、AQP3表达相关性;绘制受试者工作特征(ROC)曲线分析血清PF4及癌组织AQP1、AQP3表达对膀胱癌不良预后的预测效能。结果观察组血清PF4水平高于对照组(P<0.01)。观察组癌组织AQP1、AQP3表达均高于癌旁组织(P<0.01)。TNM分期Ⅲ~Ⅳ期、肿瘤最大径>2 cm、有淋巴结转移患者血清PF4水平与癌组织AQP1、AQP3表达分别高于TNM分期Ⅰ~Ⅱ期、肿瘤最大径≤2 cm、无淋巴结转移患者(P<0.01)。膀胱癌患者血清PF4水平与癌组织AQP1、AQP3表达呈正相关(r=0.721、0.709,P<0.01)。预后不良患者血清PF4水平与癌组织AQP1、AQP3表达量高于预后良好患者(P<0.01)。ROC曲线显示,血清PF4水平与癌组织AQP1、AQP3表达单独及联合预测膀胱癌不良预后的敏感度/特异度分别为0.885/0.833、0.654/0.864、0.962/0.742、0.923/0.955,曲线下面积分别为0.924、0.822、0.948、0.975。结论膀胱癌患者血清PF4水平及癌组织AQP1、AQP3表达与临床病理特征密切相关,可将三者联合检测作为预后预测指标。Objective To analyze correlations between platelet factor 4(PF4)level and expressions of aquaporin 1(AQP1)and aquaporin 3(AQP3)with disease progression and prognosis in patients with bladder cancer.Methods A total of 92 patients with bladder cancer between January 2016 and January 2018 were selected as observation group,and other 50 healthy persons who had undergone physical examinations at the same period were selected as control group.Serum levels of PF4 between two groups and expressions of AQP1 and AQP3 in cancerous and adjacent tissues in observation group were compared.Correlations between clinicopathological features with serum PF4,AQP1 and AQP3 in cancer tissues were analyzed in observation group.Serum levels of PF4 and expressions of AQP1 and AQP3 in cancerous tissues were compared in patients with different prognoses.Pearson correlation analysis was used to explore correlations between serum PF4 with expressions of AQP1 and AQP3 in cancerous tissues.Receiver operating characteristic(ROC)curve was drawn to analyze efficacies of serum PF4 and expressions of AQP1 and AQP3 in prediction of prognosis of patients with bladder cancer.Results Serum level of PF4 in observation group was significantly higher than that in control group(P<0.01).Expressions of AQP1 and AQP3 in cancerous tissues in observation group were significantly higher than those in adjacent tissues(P<0.01).Serum levels of PF4 and expressions of AQP1 and AQP3 in tumor tissues in patients with TNM stageⅢ-Ⅳ,tumor maximum diameter more than 2 cm and lymph node metastasis were significantly higher than those in patients with TNM stageⅠ-Ⅱ,tumor maximum diameter equal or less than 2 cm and no lymph node metastasis respectively(P<0.01).Serum level of PF4 in bladder cancer patients was positively correlated with expressions of AQP1 and AQP3 in cancerous tissues(r=0.721,0.709,P<0.01).Serum levels of PF4 and expressions of AQP1 and AQP3 in patients with poor prognoses were significantly higher than those in patients with good prognoses(P<0.01).RO
