摘  要:目的观察反流性食管炎(RE)患者胃镜表现,分析RE患者幽门螺杆菌(Hp)感染情况及影响因素。方法以2018年6月—2021年1月在本院治疗的168例RE为研究对象。观察RE患者胃镜表现,分析不同分级RE患者Hp感染率及与Hp感染程度的构成比,采用多因素Logistic回归分析寻找RE患者发生Hp感染的影响因素。结果168例中胃镜表现A级101例、B级55例、C级12例、D级0例;观察组Hp感染率为61.31%(103/168)高于对照组的21.51%(37/172)(P<0.01);Hp感染率RE A级者<B级者<C级者(P<0.05);不同分级RE患者Hp感染程度-、+++~++++构成比比较差异有统计学意义(P<0.05,P<0.01);男性、年龄>65岁、有饮酒史、有吸烟史、不良口腔卫生习惯、喜辛辣食物、暴饮暴食、有家族Hp感染史为RE患者发生Hp感染的独立危险因素(P<0.05,P<0.01)。结论RE患者Hp感染率较高,且Hp感染程度随着RE分级的增加而加重;RE患者发生Hp感染的影响因素较多,临床需针对相关危险因素实施干预,以降低患者Hp感染率。Objective To observe gastroscopic manifestations of patients with reflux esophagitis(RE)and to analyze the infection of Helicobacter pylori(HP)and its influencing factors.Methods A total of 168 patients with RE admitted between June 2018 and January 2021 were enrolled in this study.Gastroscopic manifestations of RE patients were observed,and HP infection rate and the constituent ratio of HP infection degree were analyzed among patients with different grades of RE.Influencing factors of HP infection in RE patients were explored by multivariate Logistic regression analysis.Results Among the 168 patients,there were 101 patients with grade A gastroscopic manifestation,55 patients with grade B,12 patients with grade C and 0 patients with grade D.The HP infection rate in observation group(61.31%,103/168)was significantly higher than 21.51%(37/172)in control group(P<0.01).The HP infection rates among patients with different grades of RE was the lowest in grade A,followed by grade B and grade C(P<0.05).There were significant differences in HP infection degree-and the composition ratio of+++-+++++among patients with different grades of RE(P<0.05,P<0.01).Male,age more than 65 years old,drinking history,smoking history,poor oral hygiene habits,liking spicy food,binge overeating and family history of HP infection were independent risk factors for HP infection in RE patients(P<0.05,P<0.01).Conclusion The incidence rate of HP infection in patients with RE is high,and the degree of HP infection is aggravated with the increase of RE grade.There are many influencing factors of HP infection in patients with RE.Clinical intervention should be carried out according to relevant risk factors to reduce the incidence rate of HP infection.
