结直肠癌^(18)F-FDG PET/CT代谢参数与临床病理特征的相关性

摘　　要：目的探讨结直肠癌^(18)F-FDG PET/CT代谢参数与临床病理特征的相关性。方法回顾性收集了43例术前进行过^(18)氟-氟脱氧葡萄糖(^(18)F-FDG)PET-CT检查的结直肠癌患者的临床病理资料,测量结直肠癌原发灶的最大标准化摄取值(SUV_(max))、平均标准化摄取值(SUV_(mean))、肿瘤代谢体积(tumor metabolismvolume,MTV)、糖酵解总量(total glycolysis,TLG)。比较不同临床病理特征分组间的代谢参数差异有无统计学意义,并分析各代谢参数与临床病理特征的相关性,绘制诊断效能受试者工作特征(receiver operatorcharacteristic,ROC)曲线评估PET/CT各代谢参数评估T分期的诊断效能。结果SUV_(max)、SUV_(mean)、MTV和TLG在肿瘤最大径、T分期分组间差异有统计学意义(P<0.05);SUV_(max)、SUV_(mean)在分化程度分组间差异有统计学意义(P<0.05);SUV_(max)、SUV_(mean)、MTV和TLG与肿瘤最大径、T分期呈正相关(P<0.05);SUV_(max)、SUV_(mean)和分化程度呈负相关(P<0.05);ROC曲线显示,在鉴别结直肠癌患者的T3和T4期时,SUV_(max)、SUV_(mean)、MTV和TLG的曲线下面积(area undercurve,AUC)阈值分别为0.734、0.696、0.780、0.829及15.05、0.90、9.93、79.70;SUV_(max)、SUV_(mean)、MTV和TLG以各自阈值评估结直肠癌T3、T4分期的敏感性及特异性分别为83.3%、69.6%;83.3%、65.2%;91.3%、61.1%及91.3%、61.1%。结论^(18)F-FDG PET/CT的半定量代谢参数SUV_(max)、SUV_(mean)、MTV和TLG与结直肠癌原发灶肿瘤大小、分化程度、T分期等具有一定相关性,在一定程度上反映肿瘤的部分病理特征,有助于判断肿瘤的特性及恶性程度。Objective:To explore the correlation between ^(18)F-FDG PET/CT metabolic parameters and clinical pathological characteristics of colorectal cancer.Methods:The clinicopathological data of 43 patients with colorectal cancer who underwent preoperative ^(18)F-FDG PET-CT examination were retrospectively collected,and the maximum standardized intake(SUV_(max)),average standardized intake(SUV_(mean)),tumor metabolism volume(MTV),and sugar fermentation(TLG)of colorectal cancer were collected.The differences of metabolic parameters between different clinicopathological characteristics were compared,and the correlation between metabolic parameters and the clinicopathological characteristics was analyzed.ROC curve was drawn to evaluate the diagnostic efficacy of metabolic parameters of PET/CT in evaluating T stage.Results:There were significant differences in SUV_(max),SUV_(mean),MTV and TLG between tumor maximum diameter and T stage(P<0.05);The difference of SUV_(max) and SUV_(mean) in differentiation degree was statistically significant(P<0.05);SUV_(max),SUV_(mean),MTV and TLG were positively correlated with maximum tumor diameter and T stage(P<0.05);SUV_(max) and SUV_(mean) were negatively correlated with the degree of differentiation(P<0.05);ROC curve showed that the area under curve of SUV_(max),SUV_(mean),MTV and TLG in differentiating T3 and T4 of colorectal cancer patients AUC thresholds were 0.734,0.696,0.78,0.829 and 15.05,0.90,9.93,79.70,respectively.The sensitivity and specificity of SUV_(max),SUV_(mean),MTV and TLG for T3 and T4 stages of colorectal cancer were 83.3%,69.6%,83.3%,65.2%,91.3%,61.1%and 91.3%,61.1%,respectively.Conclusion:The semi quantitative metabolic parameters SUV_(max),SUV_(mean),MTV and TLG of ^(18)F-FDG PET/CT have a certain correlation with the size,differentiation and T stage of primary colorectal cancer,reflect some pathological characteristics of the tumor to a certain extent,and help to judge the characteristics and malignancy of the tumor.
