续命通脉汤治疗急性缺血性卒中疗效及对血清S100B蛋白表达的影响

摘　　要：目的研究续命通脉汤治疗急性缺血性卒中(AIS)的临床疗效、安全性及对血清星型胶质源性蛋白(S100B)表达的影响。方法选择AIS患者201例按1∶1随机分为观察组与对照组,对照组给予急性缺血性卒中指南规范化治疗,观察组在对照组的治疗基础上加用续命通脉汤,每日3次,连服14 d。观察两组患者的治疗效果、治疗前后的美国国立卫生研究院卒中量表(NIHSS)评分、Barthel指数(BI)评分、功能综合评定量表(FCA)评分、血清S100B蛋白水平及安全性指标的变化。结果观察组在降低NIHSS评分、升高BI及FCA评分、降低S100B水平方面优于对照组,差异有统计学意义(P<0.05)。观察组总有效率为64.36%,对照组为48.00%,两组疗效比较差异有统计学意义(P<0.05);安全性指标均未见明显异常,试验中观察组发生腹泻1例,余无明显不良反应发生。结论续命通脉汤能安全、有效地改善AIS患者神经功能缺损程度,提高患者日常生活功能,其机制可能与下调血清S100B水平有关。Objective:To study the clinical efficacy,safety and effect of Xuming Tongmai Decoction on serum astroglial-derived protein(S100B)expression in the treatment of acute ischemic stroke(AIS).Methods:A total of 201 patients with AIS were selected and randomly divided into the observation and control groups according to 1∶1 ratio.The control group was given the standardized treatment of acute ischemic stroke guidelines,while the ob⁃servation group was given the treatment same as the control group,with the addition of Xuming Tongmai Decoc⁃tion,3 times/d for 14 days.The treatment effects,NIHSS scores,Barthel Index(BI)scores,FCA scores,serum S100B protein levels and safety indicators of the two groups were observed before and after treatment.Results:The observation group was better than the control group in reducing NIHSS scores,increasing BI and FCA scores,and reducing S100B levels,and the difference was statistically significant(P<0.05).The effective rate of the ob⁃servation group was 64.36%,compared with 48.00%in the control group,and there was a statistical difference in the efficacy of the two groups(P<0.05).There was no significant abnormality in the safety indexes.One case in the observation group had diarrhoea,but no significant adverse reactions occurred in the rest of the patients.Con⁃clusion:Xuming Tongmai Decoction could safely and effectively improve the degree of neurological deficits and daily life functions in patients with AIS,and the mechanism may be related to the down-regulation of serum S100B levels.
