少年儿童及成年人高眼压症非接触眼压计与Goldmann压平眼压计测量值的一致性分析

摘　　要：目的分析少年儿童及成年人高眼压症患者,非接触眼压计(NCT)与Goldmann压平眼压计(GAT)测量值的一致性。方法收集2021年11月-2022年5月医院NCT测量结果≥22mmHg、最佳矫正视力0.8、年龄≤14岁或≥18岁的患者共58眼/33人,对所有患者进行NCT测量、GAT测量、立体照相下视神经乳头分析。计算NCT与GAT结果的差值(ΔIOP)。采用Bland-Altmann散点图分析GAT与NCT间的一致性;皮尔森相关性分析探讨ΔIOP与角膜厚度的相关性。结果年龄≤14岁共36眼,完成GAT测量的20眼,GAT成功率55.56%(20眼/36眼),ΔIOP为4.04±2.76;年龄大于≥18岁22眼,GAT成功率100.00%,ΔIOP为4.60±2.63。年龄≤14岁组与年龄大于≥18岁组ΔIOP比较差异无统计学意义(t=0.673,P=5.505)。低龄组与高龄组GAT与NCT均具有较好的一致性。低龄组ΔIOP与中央角膜厚度呈明显正相关(r=0.50,P=0.023);成年人组ΔIOP与中央角膜厚度无明显相关性(r=-0.05,P=0.872)。结论少年儿童及成年人GAT与NCT均存在一定差异,且GAT与NCT均具有较好的一致性,两者间的差值受中央角膜厚度影响。Objective To analyze the consistency of non-contact tonometer(NCT)and Goldmann applanation tonometer(GAT)in children and adults with ocular hypertension.Methods A total of 58 eyes/33 patients with NCT measurement results≥22mmhg,best corrected visual acuity 0.8,age≤14 years or≥18 years were collected from November 2021 to May 2022.Intraocular pressures were measured by GAT and NCT,stereoscopic color fundus photographs were performed for optic nerve head analysis.The difference between NCT and GAT was defined asΔIOP;The consistency between GAT and NCT was analyzed by using bland Altmann scatter diagram.Pearson correlation analysis were used to analyze the correlation betweenΔIOP and central corneal thickness.Results 36 eyes aged≤14 years old,20 eyes completed gat measurement,the success rate of GAT was 55.56%(20 eyes/36 eyes),and theΔIOP was 4.04±2.76.22 eyes older than or equal to 18 years old,the success rate of GAT was 100.00%,and theΔIOP was 4.60±2.63.Age≤14 years old group and age≥18 years old group,there was no significant difference inΔIOP(t=0.673,P=5.505).The GAT and NCT of young age group and old age group had good consistency.Conclusions There are some differences between gat and NCT in children and adults,and gat and NCT have good consistency.The difference between them is affected by the central corneal thickness.
