拓扑替康联合阿霉素治疗老年复发性卵巢癌的疗效及其对卵巢血流参数的影响

摘　　要：目的探讨拓扑替康联合阿霉素治疗老年复发性卵巢癌疗效及对卵巢血流参数的影响。方法选择2018年6月—2021年6月于陕西汉中三二〇一医院确诊并收治的116例复发性卵巢癌患者,通过数字表法随机分成观察组与对照组,每组58例。对照组采取阿霉素治疗,观察组采取拓扑替康联合阿霉素治疗,治疗2个疗程。比较2组临床疗效和不良反应情况;观察并比较2组治疗前后的CA125水平、HE4水平、PSV值、RI值、PI值。结果观察组的客观缓解率以及疾病控制率均高于对照组(P<0.05);治疗前,2组CA125水平、HE4水平、PSV值、RI值、PI值差异均无统计学意义(P>0.05);治疗后,2组血清CA125、HE4水平及PSV值较同组治疗前降低(P<0.05),2组RI和PI值较同组治疗前增高(P<0.05),观察组血清CA125、HE4水平及PSV值均低于对照组(P<0.05),观察组RI和PI值均高于对照组(P<0.05);2组治疗不良反应总发生率差异无统计学意义(P>0.05)。结论拓扑替康联合阿霉素治疗老年患者复发性卵巢癌疗效较好,可改善卵巢血流参数,下调血清肿瘤标志物分子水平。Objective To investigate the effect of topotecan combined with adriamycin on recurrent ovarian cancer in elderly patients and its effect on ovarian blood flow parameters.Methods 116 patients with recurrent ovarian cancer diagnosed and treated in Hanzhong Hospital 3201 from June 2018 to June 2021 were selected and randomly divided into the observation group and control group,with 58 cases in each group.The control group was treated with adriamycin,and the observation group was treated with topotecan combined with adriamycin.Both groups were treated for 2 courses.The clinical efficacy and adverse reactions of the two groups were compared.The CA125 level,HE4 level,PSV value,RI value and PI value were detected and compared between the two groups before and after treatment.Results The objective remission rate and disease control rate of the observation group were higher than those of the control group(P<0.05).Before treatment,there was no significant difference in CA125 level,HE4 level,PSV value,RI value and PI value between the two groups(P>0.05).After treatment,the CA125 level,HE4 level and PSV value of the two groups were lower than those of the same group before treatment(P<0.05),and the values of RI and PI of the two groups were higher than those of the same group before treatment(P<0.05);the serum CA125 level,HE4 level and PSV value of the observation group were lower than those of the control group(P<0.05),while the values of RI and PI of the observation group were higher than those of the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Topotecan combined with doxorubicin has good curative effect in the treatment of elderly patients with recurrent ovarian cancer,which can improve ovarian blood flow parameters and down-regulate the levels of serum tumor markers.
