急性胰腺炎并巨大十二指肠壁内血肿1例报道

摘　　要：十二指肠壁内血肿(intramural duodenal hematoma,IDH)是一种罕见的疾病,常发生于十二指肠降段及水平段,病因多为上腹部钝性挤压伤、抗凝治疗、特发性血小板减少性紫癜等出血性疾病,偶见于内镜检查及治疗、胰腺炎。合并急性胰腺炎的IDH少有报道。IDH早期出血量少,压力不高,临床症状不明显。随着血肿体积进行性增大,可出现胆红素血症、肠梗阻及十二指肠坏死穿孔、腹膜炎、败血症等危及生命的并发症,因此,早期正确诊断和治疗极其重要。本文报道1例合并急性胰腺炎的巨大IDH。Intramural duodenal hematoma(IDH) is rare in the clinic and it often occurs in the junction of the descending and horizontal parts of the duodenum.Blunt abdominal trauma,anticoagulation therapy,or bleeding disorders are the common causes of IDH,other causes including duodenal endoscopic or various pancreatic diseases.IDH of pancreatic origin has been infrequently reported.During the early stage of IDH,the volume of hematoma in the duodenal wall is small and the clinical symptoms are not obvious.However with the volume of the hematoma increases progressively,it can be life-threatening and serious complications may occur,including bilirubinemia,upper gastrointestinal tract obstruction,duodenal perforation,peritonitis,and hematosepsis.Therefore,it is crucial to have early and correct diagnosis and treatment.This article reported a case of acute pancreatitis complicated with IDH.
