摘　　要：目的:探讨新型胃癌筛查评分系统(GCR)联合癌胚抗原(CEA)、糖类抗原(CA)199、CA724水平检测在早期胃癌(EGC)诊断中的应用价值。方法:选取50例EGC患者为EGC组,选取同期收治的50例胃溃疡(GU)患者为GU组,选取50名同期健康体检者为健康组。比较三组GCR评分和CEA、CA199、CA724水平;采用ROC曲线分析GCR、CEA、CA199和CA724对EGC的诊断价值;Spearman相关性分析GCR评分、CEA水平、CA199水平和CA724水平与EGC患者病理分期的相关性。结果:EGC组GCR评分和CEA、CA199、CA724水平均高于GU组和健康组,且GU组高于健康组,差异有统计学意义(P<0.05);ROC曲线分析显示,GCR、CEA、CA199和CA724诊断EGC的曲线下面积(AUC)分别为0.740、0.714、0.729和0.618,联合诊断AUC为0.862;50例EGC患者中,N1期15例,N2期17例,N3a期12例,N3b期6例;Spearman相关性分析可见,GCR评分、CEA水平、CA199水平和CA724水平与EGC病理分期呈正相关(r=0.806、0.736、0.656和0.662,P<0.05)。结论:GCR评分联合CEA、CA199、CA724水平检测在EGC诊断中具有较高应用价值。Objective:To observe application effects of new gastric cancer screening scoring system(GCR)combined with carcinoembryonic antigen(CEA),CA199 and CA724 level detection in diagnosis of early gastric cancer.Methods:50 patients with EGC were selected as EGC group,50 patients with gastric ulcer(GU)admitted during the same period were selected as GU group,and 50 cases with healthy physical examinations during the same period were selected as the healthy group.The GCR scores and the levels of CEA,CA199 and CA724 were compared among the three groups.The ROC curve was used to analyze the diagnostic values of GCR,CEA,CA199 and CA724 for EGC.Further,Spearman correlation analysis was used to analyze the correlation among the GCR score,CEA level,CA199 level and CA724 level and the pathological stages of the EGC patients.Results:The GCR score and the levels of CEA,CA199,and CA724 in the EGC group were higher than those in the GU group and the healthy group,those of the GU group were higher than those of the healthy group,and the differences were statistically significant(P<0.05).ROC analysis showed that the levels of the area under the curve(AUC)of GCR,CEA,CA199 and CA724 in the diagnosis of EGC were 0.740,0.714,0.729 and 0.618,separately,and that of combined AUC was 0.862.Among the 50 EGC patients,15 were in N1 stage and 17 in N2 stage,12 cases in N3a stage and 6 cases in N3b stage.Spearman correlation analysis showed that the GCR score,the CEA level,the CA199 level and the CA724 level were positively correlated with the EGC pathological stage(r=0.806,0.736,0.656 and 0.662,P<0.05).Conclusions:GCR score combined with the CEA,CA199,CA724 level detection has a high application value in the diagnosis of EGC.
