摘 要:目的探讨hs-CRP、MMP-9、S100、Fn在脑梗死出血性转化诊断中的临床意义。方法选择2019-01-2020-12商丘市第一人民医院收治的65例急性脑梗死出血患者为观察组,同期非溶栓治疗后转化后无出血的急性脑梗死患者70例为对照组。检测2组血清中hs-CRP、MMP-9、S100、Fn的表达,采用多元Logistic回归分析脑梗死危险因素,采用相关系数分析hs-CRP、MMP-9、S100与Fn在脑梗死诊断中的诊断效能及联合检测的预测意义。结果观察组与对照组MMP-9浓度[(49.91±7.3)μg/L vs(12.56±6.89)μg/L]、Fn浓度[(4827.92±732.21)μg/L vs(2564.25±853.4)μg/L]、hs-CRP水平[(14.86±3.4)mg/L vs(4.12±1.2)mg/L]、S100浓度(6.23±1.49 vs 0.087±0.026)比较,差异均有统计学意义(P<0.05)。单因素及相关系数分析显示,MMP-9、hs-CRP、S100和Fn在预测脑梗死出血转化上具有唯一性,可作为独立预测因素,综合分析的准确率更高(P<0.05)。结论hs-CRP、MMP-9、S100和Fn水平与急性脑梗死患者病情严重程度及出血风险呈正相关,联合检测具有重要预测意义。Objective To observe the clinical significance of hs-CRP,MMP-9,S100 and Fn in the diagno sis of cerebral infarction hemorrhage transformation.Methods Totally 65 patients with acute cerebral infarction complicated with cerebral infarction hemorrhage admitted to Shangqiu First People’s Hospital from January 2019 to December 2020 were selected as the observation group.Seventy patients without hemorrhagic transformation af ter non-thrombolytic treatment of infarction were served as the control group.The expressions of hs-CRP,MMP-9,S100 and Fn in the serum of the two groups were detected,multivariate Logistic regression was used to analyze the risk factors of cerebral infarction,and apply correlation coefficient analysis hs-CRP,MMP-9.The diagnostic ef ficacy of S100 and Fn and combined detection in the diagnosis of cerebral infarction were analyzed.Results MMP-9 concentration in observation group and control group((49.91±7.3)μg/L vs(12.56±6.89)μg/L),Fn con centration((4827.92±732.21)μg/L vs(2564.25±853.4)μg/L),hs-CRP level((14.86±3.4)mg/L vs(4.12±1.2)mg/L),S100 concentration(6.23±1.49 vs 0.087±0.026),the differences were statistically significant(P<0.05).Univariate and correlation coefficient analysis showed that MMP-9,hs-CRP,S100 and Fn were unique in predicting the transformation of cerebral infarction hemorrhage and can be used as an independent predictor,and the accura cy of comprehensive analysis was higher(P<0.05).Conclusion hs-CRP,MMP-9 and Fn are closely related to the prognosis of cerebral infarction,especially S-100 has the most significant effect.
