摘 要:目的探讨基于交互抑制理论的肢体康复锻炼操对于脑卒中偏瘫患者肢体运动、平衡功能和日常生活活动能力的影响。方法纳入60例脑卒中患者,其中对照组30例,治疗组30例,2组均给予常规药物治疗和早期运动康复,治疗组在此基础上以团体的形式进行脑卒中肢体康复锻炼操的学习和训练,30~40 min/次,1次/d,5次/周,共进行8周。分别于训练前后采用Brunnstrom分级法、简化Fugl-Meyer运动功能评分法(FMA)、Berg平衡量表和Barthel指数积分法对患者肢体运动功能、平衡功能和日常生活活动能力(ADL)进行评测。结果训练后治疗组和对照组肢体运动功能(上肢、下肢和手Brunnstrom分级)差异有统计学意义(P<0.05)。训练后治疗组患者上肢和下肢FMA评分分别为(53.0±13.2)分和(29.5±4.7)分,均高于对照组,差异有统计学意义(P<0.05)。与对照组相比,训练后治疗组Berg评分(46.5±9.6)和ADL评分(88.8±14.2)得分均较高,差异均有统计学意义(P<0.05)。结论应用基于交互抑制理论的脑卒中肢体康复锻炼操可减少共同运动模式的出现,有助于诱发和产生分离运动,提高患者的运动、平衡功能和日常生活自理能力,提高生存质量,同时也给患者及家属提供一个可以长期坚持的训练方法,使患者能够有意义地度过治疗的空余时间。Objective To investigate the effect of physical rehabilitation exercise based on the theory of interactive inhibition on limb movement,balance function,and activities of daily living in stroke patients with hemiplegia.Methods A total of 60 stroke patients were included,including 30 in the control group and 30 in the treatment group.Both groups were given conventional drug treatment and early exercise rehabilitation.On this basis,the learning and training of stroke limb rehabilitation exercise gymnastics were performed in a group format in the treatment group,30~40 min/time,1 time/d,5 times/week,for 8 weeks in total.Before and after training,the Brunnstrom grading method simplified Fugl-Meyer motor function score(FMA),Berg balance scale,and Barthel index score were used to evaluate patients’limb motor function,balance function,and activities of daily living(ADL).Results There were significant differences on limb motor function(Brunnstrom classification of upper limbs,lower limbs,and hands)between the treatment group and the control group after training(P<0.05).The FMA scores of the upper and lower limbs of the patients in the treatment group were(53.0±13.2)points and(29.5±4.7)points,respectively after training,both of which were higher than those of the control group,with statistically significant differences(P<0.05).Compared with the control group,the scores of the Berg score(46.5±9.6)and ADL score(88.8±14.2)were both higher in the treatment group after training,and the differences were statistically significant(P<0.05).Conclusion The application of stroke limb rehabilitation exercises based on the interactive inhibition theory can reduce the appearance of common movement patterns,help to induce and produce dissociative movements,improve patients’movement,balance function,and self-care ability in daily life,and improve the quality of life.Patients and their families provide a training method that can be adhered to for a long time so that patients can spend their spare time meaningfully.
