中医耳穴压籽法对晚期前列腺癌患者癌性疼痛、睡眠障碍及细胞免疫功能的影响

摘　　要：目的探讨中医耳穴压籽法对晚期前列腺癌患者癌性疼痛、睡眠障碍及细胞免疫功能的影响。方法选取2017年1月至2019年12月浙江省台州医院治疗的晚期前列腺癌患者84例,采用随机数字表法分为干预组与对照组,每组各42例。对照组予常规干预措施。干预组在对照组基础上加中医耳穴压籽法治疗,两组均干预3个月。评估并比较两组干预前后癌性疼痛程度、睡眠情况及细胞免疫功能的变化情况。结果干预3个月后,干预组与对照组VAS评分[(2.73±0.46)分、(3.68±0.61)分]和SRSS评分[(18.15±2.49)分、(22.72±1.67)分]较干预前[(5.64±0.89)分、(5.72±0.97)分,(31.73±4.28)分、(31.42±4.18)分]显著下降(P<0.05或P<0.01),且干预后干预组下降幅度更显著(P<0.05);干预组与对照组血清CD4^(+)水平[(43.72±4.17)%、(39.13±4.04)%]和CD4^(+)/CD8^(+)比值[(27.82±3.45)、(31.92±3.67)]较干预前[(36.02±3.76)%、(35.72±3.81)%,(34.08±3.92)、(34.12±4.02)]显著上升,CD8^(+)水平[(1.57±0.29)%、(1.22±0.21)%]较干预前[(1.06±0.16)%、(1.05±0.18)%]显著下降(P<0.05或P<0.01),且干预后干预组变化幅度更显著(P<0.05)。结论中医耳穴压籽法用于晚期前列腺癌患者不仅可减轻癌性疼痛程度,改善睡眠状况,而且可调节外周血T淋巴细胞失调,增强细胞免疫功能。Objective To explore the effect of auricular point acupressure(APA)in traditional Chinese medicine(TCM)on cancer-related pain(CRP),sleep disorder and cellular immune function in patients with advanced prostate cancer.Methods A total of 84 patients with advanced prostate cancer treated in Taizhou Hospital of Zhejiang Province affiliated to Wenzhou Medical University from January 2017 to December 2019 were selected and divided into the intervention group and the control group by a random number table with 42 cases each.The control group received conventional intervention measures.On the basis of the conventional intervention same as the control group,the intervention group was treated with APA in TCM,and both groups were intervened for 3 months.The changes in the degree of CRP,sleep quality and cellular immune function before and after intervention were assessed and compared between the two groups.Results After 3 months of intervention,visual analogue scale(VAS)scores([2.73±0.46]points,[3.68±0.61]points)and SRSS scores([18.15±2.49]points,[22.72±1.67]points)of the intervention group and the control group significantly decreased than before intervention([5.64±0.89]points,[5.72±0.97]points,[31.73±4.28]points,[31.42±4.18]points)(P<0.05 or P<0.01).The decrease after intervention in the intervention group was more significant(P<0.05).At the same time,the serum CD4^(+)level of the intervention group and the control group([43.72±4.17]%,[39.13±4.04]%)and the CD4^(+)/CD8^(+)ratio([27.82±3.45],[31.92±3.67])increased significantly than before intervention([36.02±3.76]%,[35.72±3.81]%,[34.08±3.92],[34.12±4.02]).CD8^(+)levels([1.57±0.29]%,[1.22±0.21]%)significantly decreased than before intervention([1.06±0.16]%,[1.05±0.18]%)(P<0.05 or P<0.01),and the change after intervention in the intervention group was more significant(P<0.05).Conclusion The APA in TCM can not only reduce the degree of CRP and improve sleep conditions,but also regulate the imbalance of peripheral blood T lymphocytes and enhance cellular im
