富血小板纤维蛋白联合GTR对慢性牙周炎牙槽骨垂直吸收患牙牙周软硬组织的修复效果研究

摘　　要：目的:探讨富血小板纤维蛋白(Platelet-rich fibrin,PRF)联合引导组织再生术(Guided tissue regeneration,GTR)治疗慢性牙周炎对牙槽骨垂直吸收患牙的牙周袋深度、骨密度、骨高度和组织修复再生的影响。方法:选取2018年6月-2020年6月笔者医院就诊的126例慢性牙周炎患者,按照随机数字表法将其分为研究组和对照组,每组63例。研究组采用PFR联合GTR治疗,对照组单纯采用GTR治疗。观察分析两组治疗后的临床疗效,治疗前后血清炎症因子水平、牙周袋深度、牙槽骨密度和牙槽骨高度,术前及术后6个月骨缺损处位点的牙周探诊深度(Probing depth,PD)、临床附着水平(Attachment level,AL)和Mazza出血指数(Bleeding index,BI)。结果:治疗后,两组患者血清白细胞介素-6(Including interleukin-6,IL-6)、白细胞介素-8(Including interleukin-8,IL-8)、肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)水平明显低于治疗前,且研究组低于对照组,治疗后两组牙周袋深度均明显小于治疗前,且研究组小于对照组,治疗后牙槽骨密度明显高于治疗前,且研究组高于对照组,术后1个月、3个月和6个月两组PD、AL及BI指标明显低于术前,且研究组PD和AL指标明显低于对照组,研究组的治疗总有效率明显高于对照组,差异均有统计学意义(P<0.05)。结论:PFR联合GTR治疗慢性牙周炎可以有效促进组织再生,提高临床疗效,降低血清炎症因子水平,提高牙槽骨密度,减小牙周袋深度,改善牙周情况。Objective To investigate the eff ects of platelet-rich fi brin(PRF)combined with guided tissue regeneration(GTR)in the treatment of chronic periodontitis on periodontal pocket depth,bone mineral density,bone height and tissue repair and regeneration of teeth with vertical absorption of alveolar bone.Methods 126 patients with chronic periodontitis were selected from June 2018 to June 2020.According to random table number,patients were divided into study group and control group,63 cases each group.PFR combined GTR therapy was performed in study group and simple GTR therapy was performed in control group.After treatment,serum infl ammatory factor levels,periodontal pocket depth,alveolar bone density and alveolar height were observed before and after treatment.Probing depth(PD),attachment level(AL)and Mazza hemorrhage index(BI)were observed before and 6 months postoperatively.Results Two groups after treatment in patients with serum IL-6,IL-8 and TNF-αalpha level signifi cantly lower than before the treatment,and the team is lower than the control group,two groups after treatment were signifi cantly less than before the treatment,periodontal pocket depth is less than the control group,and group alveolar bone mineral density were significantly higher than those before treatment,after treatment and the team is higher than the control group,two groups of PD after 1 month,3 months,and 6 months,and BI index was lower than that before the operation,and the team of PD and AL index signifi cantly lower than the control group,treatment group total eff ective rate is signifi cantly higher than the control group,the diff erence is statistically signifi cant(P<0.05).Conclusion PFR combined with GTR in the treatment of chronic periodontitis can eff ectively promote tissue regeneration,improve clinical effi cacy,reduce the level of serum infl ammatory factors,improve alveolar bone density,reduce the depth of periodontal pocket,and improve the periodontal condition.
