摘 要:目的通过观察不同基质蛋白酶抑制剂[2%氯己定、5%表没食子儿茶素没食子酸酯(EGCG)、0.5 mol/L碳二亚胺盐酸盐(EDC)]预处理对龋病影响牙本质(CAD)粘结效果的影响,以期为临床提供依据。方法240颗拔除的有龋离体牙,随机分为A组、B组、C组,各80颗;另选取80颗无龋离体牙作为D组。A组采用2%氯己定预处理、B组采用5%EGCG预处理、C组采用0.5 mol/L EDC预处理、D组采用去离子水预处理。对比四组牙体粘结强度及断裂类型。结果四组粘结强度由高至低依次为A组、B组、C组、D组,差异有统计学意义(P<0.05)。A组混合破坏率11.25%低于B组的38.75%、C组的76.25%及D组的52.50%,界面破坏率81.25%高于B组的11.25%、C组的13.75%及D组的16.25%,内聚破坏率7.50%低于B组的50.00%及D组的31.25%,差异有统计学意义(P<0.05)。结论不同基质金属蛋白酶抑制剂预处理方案对龋病影响牙本质粘结强度的影响不同,2%氯己定预处理显著提高牙体粘结强度,可在临床牙体疾病的治疗中推广使用。Objective To observe the effect of different matrix protease inhibitors[2%chlorhexidine,5%epigallocatechin gallate(EGCG),0.5 mol/L carbodiimide hydrochloride(EDC)]pretreatment on dentin bonding in caries-affected dentin(CAD),so as to provide basis for clinical practice.Methods A total of 80 isolated teeth with caries were randomly divided into group A,group B,and group C,with 80 teeth in each group.Another 80 caries-free isolated teeth were selected as group D.Group A was pretreated with 2%chlorhexidine,group B was pretreated with 5%EGCG,group C was pretreated with 0.5 mol/L EDC,and group D was pretreated with deionized water.The bonding strength and fracture type of dentin were compared among the four groups.Results The bonding strength of the four groups were group A,group B,group C,group D from high to low,and the difference was statistically significant(P<0.05).The mixed failure rate 11.25%of group A was lower than 38.75%of group B,76.25%of group C and 52.50%of group D;the interface failure rate 81.25%of group A was higher than 11.25%of group B,13.75%of group C and 16.25%of group D;the cohesive failure rate 7.50%of group A was lower than 50.00%of group B and 31.25%of group D;all the differences were statistically significant(P<0.05).Conclusion Different matrix metalloproteinase inhibitor pretreatment schemes have different effects on dentin bonding in caries-affected dentin.2%chlorhexidine pretreatment can significantly improve the bonding strength of dentin,which can be widely promoted and used in the treatment of clinical dental diseases.
