摘 要:目的针对神舟十二号航天员在轨时间长、面临的挑战增加、航天员卫勤保障要求高,探索舱前医疗救护中心肺复苏的新模式。方法神舟十二号主着陆场医疗队对东风主着陆场进行现场调研,总结分析神舟5号到11号着陆场救治经验,荟萃研究国内外相关文献,探索返回舱前心肺复苏面临新挑战以及应对策略。结果舱前心肺复苏面临的挑战包括:复苏对象的特殊性、复苏环境的不确定性、伤情的复杂性,这要求舱前心肺复苏模式应因人而异、因地制宜、因病而为地运用标准化、多元化、个体化方法进行心肺复苏。结论航天员返回舱前心肺复苏不应局限于“胸路”,应适时开通“腹路”,要建设专业的舱前急救复苏团队以及应用舱前心肺复苏装备。要注重舱前心肺复苏的特殊性、实用性和可操作性,为航天员救护筑起最安全,最可靠的保障线。Objective In view of the long time in orbit of the Shenzhou 12 astronauts,the increasing challenges they face,and the high requirements for astronauts'medical support,to explore a new model of pulmonary resuscitation in the pre-vessel medical rescue center.Methods The Shenzhou 12 main landing site medical team conducted an on-site investigation on the Dongfeng main landing site,summarized and analyzed the rescue experience of Shenzhou 5 to 11 landing sites,meta-studied relevant domestic and foreign literature,and explored the new challenges and solutions for cardiopulmonary resuscitation before returning to the cabin.Strategy.Results The challenges faced by pre-cabin cardiopulmonary resuscitation include:the particularity of the resuscitation target,the uncertainty of the resuscitation environment,and the complexity of the injury.This requires that the precabin cardiopulmonary resuscitation mode should be different from person to person,local conditions,and use according to disease.Standardized,diversified and individualized methods for cardiopulmonary resuscitation.Conclusion Before the astronauts return to the cabin,CPR should not be limited to the"thoracic route",the"abdominal route"should be opened at the appropriate time,and a professional pre-cabin emergency resuscitation team and the application of pre-cabin CPR equipment should be built.We must pay attention to the particularity,practicability and maneuverability of cardiopulmonary resuscitation in front of the cabin,and build the safest and most reliable support line for the rescue of astronauts.
