不同材质模拟穿刺模型在超声介入教学中的应用研究

摘　　要：目的通过对比3种不同材质的模拟穿刺模型在超声介入穿刺实践教学中的应用效果,来观察这3种模型在教学中的教学效果与可行性。方法选取2020年9月—2021年9月在黑龙江省医院规培的临床医学专业学生60名,随机分为商用蓝胶组、自制明胶组和自制豆腐组,各20名,并在进行实践操作前进行时长为20 min的超声引导穿刺讲座培训,且根据其所在组别应用模拟穿刺模型进行超声引导下臂丛神经阻滞操作,时间为4学时,后带教老师根据这3组学生在实操时的显影清晰度、模型仿真度、图像识别度以及模型的耐用性和易清洁性和在得知模型价格后学生选择的情况来对这3种模型教学的应用效果进行评价分析。结果3组模拟穿刺模型在实操时针尖及针道的显影清晰度上以及图像的识别度比较,差异无统计学意义(P>0.05)。穿刺模型仿真度、穿刺模型耐用性及穿刺模型易清洁性的评分上,商用蓝胶模型与自制明胶模型及自制豆腐模型相比较,差异有统计学意义(P<0.05);且3组学生在得知3种模型的价格后,有75%的学生选择自制明胶模型,10%的学生选择商用蓝胶模型,5%的学生选择自制豆腐模型。结论模拟穿刺模型的选择应用需与教学需求、经费预算以及实操效果等多方面因素相结合,在需要自主设计的教学穿刺模型中,自制明胶模型具有较大优势。Objective To observe the teaching effect and feasibility of these three models in teaching by comparing the application effects of three simulated puncture models with different materials in the practice teaching of ultrasonic interventional puncture,so as to provide reference for future practical teaching.Methods A total of 60 clinical medical students who were trained in Heilongjiang Provincial Hospital from September 2020 to September 2021 were selected and randomly divided into a commercial blue glue group,a homemade gelatin group,and a homemade tofu group,with 20 students in each group.Before the practical operation,a 20 min ultrasound-guided puncture lecture training was conducted,and a simulated puncture model was used to perform the ultrasound-guided brachial plexus block operation according to their group for 4 h.Afterwards,the teacher taught the three groups of students according to the development clarity,model simulation degree,image recognition degree,durability and easy cleaning of the model and the students'choice after knowing the model price.The application effect of model teaching was evaluated and analyzed.Results There was no statistically significant difference in the scores of the three groups of simulated puncture models in the sharpness of the needle tip and the needle track and the recognition of the image during the actual operation(P>0.05).In the scores of puncture model simulation degree,puncture model durability,and puncture model easy to clean,the commercial blue plastic model was compared with the homemade gelatin model and the homemade tofu model,and the difference was statistically significant(P<0.05).And after learning the prices of the three models,75%of the students in the three groups chose the self-made gelatin model,10%of the students chose the commercial blue plastic model,and 5%of the students chose the homemade tofu model.Conclusion The selection and application of the simulated puncture model should be combined with many factors such as teaching needs,budget and practica
