摘 要:目的调查重症监护室(ICU)护士对患者早期活动的知识、信念、行为(后简称知信行)现状及培训需求,并分析其影响因素,为护理管理者制定干预措施提供参考依据。方法采用一般资料调查表、ICU护士对患者早期活动的知信行问卷、培训需求调查表,对重庆市8所综合性三级甲等医院的436名ICU护士进行调查。结果ICU护士对患者早期活动的知识得分为(8.25±1.86)分,信念得分为(31.46±3.72)分,行为得分为(21.90±5.90)分。床旁示教、知识培训班、康复科人员指导是护理人员希望的主要培训途径。多元线性回归分析显示,接受过培训、培训意愿是ICU护士对患者早期活动知识、信念的影响因素(P<0.01);教学医院、接受过培训、培训意愿、早期活动相关规范、多学科共同查房是ICU护士对患者早期活动行为的影响因素(P<0.01)。结论管理者应制定针对性的培训计划,借鉴教学医院的管理模式,注重学科间的沟通与联系,鼓励多学科共同查房,促进ICU患者早期活动的安全、规范化开展。Objective To investigate the knowledge,belief,behavior status and training needs of ICU nurses about patients′early activities,and analyze their influencing factors to provide reference for nursing managers to formulate intervention measures.Methods A survey was conducted among 436 ICU nurses in 8 comprehensive tertiary hospitals in Chongqing by using general information questionnaire,ICU nurses′knowledge,belief and behavior questionnaire on patients′early activities,and training needs questionnaire.Results The scores of ICU nurses′knowledge of patients′early activities were(8.25±1.86)points,their belief scores were(31.46±3.72)points,and their behavior scores were(21.90±5.90)points.Bedside teaching,knowledge training courses,and the guidance of personnel in the rehabilitation department were the main training methods expected by nursing staff.Multiple linear regression analysis showed that receiving training and training willingness were the influencing factors of ICU nurses′knowledge and belief in patients′early activities(P<0.01).Teaching hospital,received training,willingness to train,norms related to early mobility,and multidisciplinary rounds were the influencing factors of ICU nurses on patients′early mobility behavior(P<0.01).Conclusion Managers should formulate targeted training plans,learn from the management model of teaching hospitals,pay attention to the communication and connection between disciplines,encourage multi-disciplinary joint rounds,and promote the safe and standardized development of early activities of ICU patients.
