青光眼滤过泡针刺分离患者治疗前后视功能相关生存质量的研究

摘　　要：目的评估滤过泡针刺分离对青光眼患者生活质量相关的视觉指标的影响。方法连续性选取2013年1月至2019年1月110例(110只眼)青光眼滤过手术患者在门诊随访中行滤过泡针刺分离治疗,记录滤过泡针刺分离治疗前后治疗眼最佳矫正视力(BCVA)(LogMAR)、双眼最佳矫正视力(LogMAR)、眼压、应用青光眼药物情况;应用视功能相关生活质量量表-25(NEI VFQ-25)进行生活质量评估,并与治疗眼BCVA、双眼BCVA、治疗眼视野MD值进行相关性分析。结果110例无功能性滤过泡患者行滤过泡针刺分离治疗后3个月,治疗眼眼压(15.54±2.68)mm Hg,较治疗前眼压(30.50±8.63)mm Hg显著降低,差异有统计学意义(P<0.05);双眼BCVA(0.40±0.31),较治疗前双眼BCVA(0.42±0.31)显著提高,差异有统计学意义(P<0.05);治疗眼视野MD(11.61±5.98)dB,较治疗前视野MD(11.77±6.06)dB显著降低,差异有统计学意义(P<0.05);NEI VFQ-25总得分平均值为(70.09±18.08),显著高于治疗前的(69.67±18.45),差异有统计学意义(P<0.05)。NEI VFQ-25总得分平均值与双眼BCVA(r=0.603,P<0.05)、治疗眼BCVA(r=0.420,P<0.05)呈正相关,与视野MD值(r=0.945,P<0.05)呈负相关。结论青光眼滤过泡针刺分离治疗可显著降低眼压,改善患者视功能,提高患者生活质量。视功能相关生存质量量表可用于滤过泡针刺分离治疗后患者的病情评估,在一定程度上反应青光眼术后患者的生活质量。Objective To evaluate the effects of acupuncture separation of filtering bleb on visual indicators related to life quality of patients with glaucoma.Methods A total of 110 patients(110 eyes)who underwent glaucoma filtration surgery in our hospital from January 2013 to January 2019 were enrolled in the study,who received acupuncture separation of filtering bleb during outpatient follow-up.The best corrected visual acuity(BCVA),binocular LogMAR,intraocular pressure,and application of glaucoma drugs were recorded before and after acupuncture separation of filtering bleb.National Eye Institute Visual Function Questionnaire-25(NEI VFQ-25)were used to evaluate the quality of life,and correlation analysis was performed on BCVA,binocular BCVA,and MD value of treated eye field.Results At 3 months afte the 110 patients with non-functional filtering bleb were treated by acupuncture separation of filtering bleb,the intraocular pressure was(15.54±2.68)mm Hg,which was significantly lower than that[(30.50±8.63)mm Hg]before treatment(P<0.05).Binocular BCVA was(0.40±0.31)mm Hg,which was significantly higher than that[(0.42±0.31)mm Hg]before treatment(P<0.05).MD of visual field of treated eyes was(11.61±5.98)dB,which was significantly lower than that[(11.77±6.06)dB]before treatment(P<0.05).The mean total scores of NEI VFQ-25 were(70.09±18.08)points,which were significantly higher than those[(69.67±18.45)]before treatment(P<0.05).Moreover the mean total scores of NEI VFQ-25 were positively correlated with binocular BCVA and BCVA of treated eyes(P<0.05),however,which were negatively correlated with MD value of visual field(P<0.05).Conclusion Acupuncture separation of filtering bleb in treatment of glaucoma can significantly reduce intraocular pressure,improve the visual function of patients,and improve the quality of life of patients.The NEI VFQ-25 can be used to evaluate the condition of patients after acupuncture separation of filtering bleb,which to a certain extent reflects the quality of life of patients after surgery f
