摘　　要：目的探讨老年食管癌患者术前衰弱现状并分析其影响因素。方法选取2020年5月至2021年1月于镇江市4所综合医院待手术的176例≥60岁的老年食管癌患者为研究对象,采用一般资料调查表、Fried衰弱表型、简版老年抑郁量表、Barthel指数问卷及老年营养风险指数进行调查,分析老年食管癌患者术前衰弱的影响因素。结果176例老年食管癌患者中衰弱发生率为38.6%(68/176),其中疲乏(83例,47.2%),握力低(80例,45.4%),自然体重下降(62例,35.2%)是发生最多的衰弱指标。食管癌分期、抑郁、自评健康状况及营养风险为老年食管癌患者术前发生衰弱的危险因素(P<0.05)。结论老年食管癌患者的术前衰弱状况不容乐观,食管癌本身及其对患者造成的身心变化是导致其衰弱的关键因素,专科护理人员应重视对老年癌症患者术前衰弱的筛查,及时采取多领域的针对性综合干预策略,缓解老年癌症患者的衰弱现状。Objective To investigate the preoperative debilitation status in elderly patients with esophageal cancer,and to explore its influencing factors.Methods A total of 176 elderly patients with esophageal cancer aged 60 and above who were treated by surgery in 4 general hospitals in Zhenjiang city from May 2020 to January 2021 were enrolled in the study.The general data questionnaire,the Fried frailty phenotype assessment scale,the simplified geriatric depression scale,the Barthel index questionnaire and the aged nutritional risk index were used to analyze the factors influencing preoperative frailty in elderly patients with esophageal cancer.Results The incidence of frailty was 38.6%(68/176)in 176 patients undergoing elective surgery,in which,fatigue(83 cases,47.2%),decline of grip strength(80 cases,45.4%)and decline of natural body mass(62 cases,35.2%)were the three most common frailty indexes.Moreover esophageal cancer stage,depression,self-rated health status,and nutritional risk were risk factors for preoperative weakness in elderly patients with esophageal cancer(P<0.05).Conclusion The preoperative frailty of aged patients with esophageal cancer is not optimistic,and esophageal cancer itself and the physical and mental changes are the key factors,therefore,it necessary to take targeted comprehensive intervention measures to improve the debilitating status of aged cancer patients.
