摘 要:目的 分析山东省淄博市2017—2020年8~10岁儿童碘营养状况,为采取针对性的防治措施提供科学依据。方法 2017—2020年,采用分层多阶段抽样方法,选择淄博市的7个碘缺乏区县为调查点,采集8~10岁儿童食用盐盐样和尿样进行碘含量检测,B超法检测儿童甲状腺容积并计算甲状腺肿大率。结果 2017—2020年共采集8~10岁儿童食用盐碘5 889份,食用盐盐碘总体中位数为23.50 mg/kg。非碘盐率6.27%,碘盐覆盖率93.73%,碘盐合格率92.17%,合格碘盐食用率86.40%,不同年份食用盐碘盐覆盖率、碘盐合格率和合格碘盐食用率差异有统计学意义(χ^(2)=83.70,84.29,36.28,P均<0.05)。2017—2020年共采集8~10岁儿童尿样5 283份,总体尿碘中位数为193.60μg/L,不同年份间尿碘中位数相比,差异有统计学意义(H=5.38,P<0.05)。共对2 221人进行了甲状腺检查,甲状腺肿大率为2.07%,不同年龄、性别和年份之间8~10岁儿童甲状腺肿大率相比,差异无统计学意义(χ^(2)=0.78,0.37,0.85,P均>0.05)。结论 淄博市2017—2020年碘盐覆盖率、碘盐合格率和合格碘盐食用率均呈下降趋势,儿童甲状腺肿大率在国家消除碘缺乏病标准范围内,8~10岁儿童碘营养总体处于适宜水平,尿碘水平偏高群体应引起关注。Objective To analyze the iodine nutrition status of children aged from 8 to 10 old years in Zibo City, Shandong Province from 2017 to 2020,and to provide evidence for prevention and treatment of the disease.Methods Seven iodine-deficient districts and counties in Zibo City were selected as survey sites by stratified multi-stage sampling method.Edible salt samples and urine samples from children aged 8 to 10 years old were collected for iodine content detection.Thyroid volume of children was determined by B-ultrasound method, and the rate of goiter was calculated.Results A total of 5 889 samples of edible iodine salt were collected from children aged 8 to 10 years old from 2017 to 2020,and the overall median of edible salt iodine was 23.50 mg/kg.The rate of non-iodized salt was 6.27%;the coverage rate of iodized salt was 93.73%;the qualified rate of iodized salt was 92.17%,and the consumption rate of iodized salt was 86.40%.There were statistically significant differences in the coverage rate, qualified rate and the consumption rate of qualified iodized salt in different years(χ~2=83.70,84.29,36.28,P<0.05).A total of 5 283 urine samples from children aged 8 to 10 years old were collected from 2017 to 2020,and the median of total urinary iodine was 193.60 μg/L.The median of urine iodine concentrations among different years was statistically significant(H=5.38,P<0.05).A total of 2 221 children were examined by thyroid ultrasound, and the rate of thyroid enlargement was 2.07%.There was no significant difference in the rate of thyroid enlargement among children aged 8 to 10 years old in different ages, genders and years(χ~2=0.78,0.37,0.85,P>0.05).Conclusions The iodized salt coverage rate and the qualified iodized salt consumption rate of Zibo City showed a downward trend from 2017 to 2020,the goiter rate of children was within the scope of the national standard for the elimination of iodine deficiency disorders, the iodine nutrition of children aged 8 to 10 years old was generally at an appropriate level, but the
