切开法重睑术在埋线法重睑术后修复中的应用

摘　　要：目的探讨埋线法重睑术后形态不满意者行切开法重睑术进行修复的效果及安全性。方法埋线法重睑术后形态不满意患者15例,均行切开法重睑术进行修复。观察切口愈合情况,感染、血肿、睑球分离及眼睑闭合不全等并发症发生情况。术后随访3~6个月,观察重睑皱襞线弧度、流畅度、对称性及瘢痕增生情况。结果15例手术时间(1.26±0.15)h,术中出血量(2.00±0.78)mL,术后切口均一期愈合,均未发生感染、血肿、睑球分离及眼睑闭合不全等并发症。术后随访3~6个月,患者睫毛上翘,重睑皱襞线弧度自然、线条流畅、双侧对称,较术前明显改善,未再次出现重睑皱襞线变浅、变窄的情况,闭眼时无明显阶梯样畸形。1例术后2个月出现瘢痕增生,外用抗瘢痕药物后瘢痕增生明显好转,未影响重睑皱襞线弧度。结论埋线法重睑术后形态不满意者行切开法重睑术进行修复后重睑形态改善明显,并可稳定维持,效果良好。Objective To investigate the efficacy and security of incision double-eyelid blepharoplasty in repairing the failed buried suture double-eyelid blepharoplasty.Methods Fifteen patients were performed incision double-eyelid blepharoplasty to repair the failed buried suture double-eyelid blepharoplasty.The incision healing,infection,hematoma,isolation of eyelid from eyeball,and hypophasis were observed.The patients were followed up for 3 to 6 months to observe radian and smoothness of double eyelid line,bilateral symmetry,and scar.Results The operation lasted(1.26±0.15)h,and the intraoperative blood loss was(2.00±0.78)mL.All patients got primary healing,with no infection,hematoma,isolation of eyelid from eyeball,or hypophasis.The 3-to 6-month follow-up survey showed upturned eyelashes,natural double-eyelid line radian,smooth lines,and symmetry between two eyes,with no shallowing and narrowing of double-eyelid line,no obvious ladder-like deformity when the eyes were closed.One patient developed hypertrophic scar in 2 months after operation,which was significantly improved without affecting the double eyelids after application of anti-scar drugs for 6 months.Conclusion The incision double-eyelid blepharoplasty can greatly improve the anatomical structure of double eyelid,and is beneficial to the maintenance of the double eyelid effect.
