针刺经筋结点配合功能性电刺激脚踏车治疗对急性缺血性脑梗死后下肢功能障碍患者的疗效

摘　　要：目的:观察针刺经筋结点配合功能性电刺激(Functional Electrical Stimulation,FES)脚踏车治疗急性缺血性脑梗死后下肢功能障碍的临床疗效。方法:将70例患者以随机数字表法分成治疗组以及对照组,各35例。对照组患者采用常规针刺配合FES脚踏车治疗,而治疗组患者给予针刺经筋结点配合FES脚踏车治疗,比较两组患者干预前后下肢运动功能简式Fugl-Meyer评分法(Fugl-Meyer Assessment Scale,FMA)评分、Berg平衡量表(Berg Balance Scale,BBS)评分、功能性步行量表(Functional Ambulation Category Scale,FAC)评分、改良Barthel指数量表(Modified Barthel Index,MBI)评分等。结果:经过4周的治疗,两组FAC评分较治疗前均有显著提高(P<0.05),两组间差异无统计学意义(P>0.05);治疗结束后,两组患者的FMA、BBS、MBI评分均较治疗前有所改善(P<0.05),且治疗组明显优于对照组,两组间差异有统计学意义(P<0.05)。结论:急性缺血性脑梗死后下肢功能障碍采用FES脚踏车配合针刺经筋结点治疗的疗效显著,经过治疗,患者的步行能力以及下肢的平衡能力都有所增强,日常生活活动能力有所提高。Objective:To observe the clinical effects of acupuncture at meridian sinew nodal points plus FES cycling on lower extremity dysfunction after acute ischemic cerebral infarction.Methods:70 cases were divided into the treatment group and the control group by the random number table method,with 35 cases in each group.Patients in the control group were treated by conventional acupuncture plus FES cycling,while patients in the treatment group were treated by acupuncture at meridian sinew nodal points plus FES cycling.Results:After 4 weeks of treatment,the FAC scores in the two groups were significantly higher than those before treatment(P<0.05),but there was no statistically significant difference between the two groups(P>0.05).After treatment,the FMA score,BBS score,and MBI score in the two groups were improved compared with those before treatment(P<0.05),and the above scores in the treatment group were significantly better than the control group(P<0.05).Conclusion:Acupuncture at meridian sinew nodal points plus FES cycling is effective on lower extremity dysfunction after acute ischemic cerebral infarction,can significantly enhance the patient’s walking ability and balance function,and improve activities of daily living.
