川芎茶调散加减联合常规西药治疗偏头痛的疗效及对血清降钙素基因相关肽水平的影响

摘　　要：目的:观察川芎茶调散加减联合常规西药治疗偏头痛的临床疗效及对血清降钙素基因相关肽(Calcitonin Gene-Related Peptide,CGRP)水平的影响。方法:选取2019年3月-2020年6月收治的72例偏头痛患者进行临床研究,按随机数字表法分为两组(各36例),治疗组采用川芎茶调散加减联合常规西药治疗,对照组仅给予常规西药治疗,连续治疗4周后,比较两组临床疗效、中医证候评分、头痛发作情况,并检测血清CGRP、内皮素-1(Endothelin-1,ET-1)水平。结果:治疗组治疗后临床总有效率显著高于对照组(P<0.05);治疗前,两组中医证候评分比较差异无统计学意义(P>0.05),两组治疗后中医证候评分降低,治疗组低于对照组(P<0.05);两组治疗前头痛发作次数、头痛程度、每次发作时间差异无统计学意义(P>0.05),两组治疗后发作次数、头痛程度、每次发作时间较治疗前显著改善,治疗组改善效果优于对照组(P<0.05);两组治疗前血清CGRP、ET-1水平差异无统计学意义(P>0.05),两组治疗后血清CGRP、ET-1水平显著下降,治疗组低于对照组(P<0.05)。结论:采用川芎茶调散加减联合常规西药治疗偏头痛,与仅给予常规西药治疗比较,有利于提高治疗效果,改善临床症状,减轻头痛程度,降低头痛发作次数和发作持续时间,这可能与调节血清CGRP和内皮素有关,对临床治疗偏头痛有重要价值。Objective:To observe the clinical efficacy of Chuanxiong Chatiao San(川芎茶调散)plus conventional Western medicine on migraine and serum CGRP level.Methods:72 cases from March 2019 to June 2020 were selected for clinical research,and were divided into 2 groups(36 cases for each)according to the random number table method.The treatment group was treated with Chuanxiong Chatiao San plus conventional Western medicine.The control group was given conventional Western medicine only.Results:The total efficiency in the treatment group after treatment was significantly higher than that in the control group(P<0.05).After treatment,TCM syndromes scores in the groups were decreased,and in the treatment group was lower than the control group(P<0.05).After treatment,the headache attacks times,headache severity,and the duration of headache attack in the groups were all significantly ameliorated compared with before treatment,and the amelioration in the treatment group was better than the control group(P<0.05).After treatment,serum CGRP and ET-1 levels in the groups were decreased significantly,and in the treatment group was lower than the control group(P<0.05).Conclusion:Chuanxiong Chatiao San plus conventional Western medicine on migraine is beneficial to improve the treatment effect,ameliorate clinical symptoms,and reduce headache severity,headache attacks times and duration of headache attacks.It may be related to the regulation of serum CGRP and endothelin,which has great value in clinical treatment of migraine.
