探讨内镜黏膜切除术治疗大肠广基息肉在临床中的应用价值及SDS评分研究

摘　　要：目的研究内镜黏膜切除术治疗大肠广基息肉在临床中的应用价值及SDS评分。方法方便选取大埔县人民医院消化内科2019年3月-2021年2月收治的256例大肠广泛息肉患者为研究对象。根据治疗方法的不同,将128例接受内镜黏膜切除术的患者作为研究组,其余128例接受常规治疗的患者作为对照组,观察两组患者临床疗效,并发症发生率及SDS评分,手术时间及术后饮食时间。结果研究组总有效率98.44%高于对照组88.28%,两组比较差异有统计学意义(χ^(2)=10.648,P<0.05)。两组患者并发症发生率对比,研究组2.34%低于对照组12.50%,差异有统计学意义(χ^(2)=9.608,P<0.05)。两组患者治疗前SDS评分比较,差异无统计学意义(t=0.514,P>0.05);治疗后,研究组SDS评分低于对照组,差异有统计学意义(t=77.450,P<0.05)。研究组、对照组手术时间及术后饮食时间比较,差异有统计学意义(t=37.300、34.013,P<0.05)。结论内镜黏膜切除术对治疗大肠广基息肉有效,有助于患者自身治疗的质量控制,对患者进行治疗指导具有较高的临床价值,可推广至临床实践。Objective To study the clinical application value and SDS score of endoscopic mucosal resection for the treatment of large bowel polyps.Methods 256 patients with extensive polyps of the large intestine admitted to the Department of Gastroenterology,Dapu County People's Hospital from March 2019 to February 2021 were conveniently selected as the research objects.According to the different treatment methods,128 patients who underwent endoscopic mucosal resection were used as the study group,and the remaining 128 patients who received conventional treatment were used as the control group.Observed the clinical efficacy,complication rate and SDS score,operation time and postoperative diet time of the two groups of patients.Results The total effective rate of the study group was 98.44%higher than that of the control group 88.28%,and the difference between the two groups was statistically significant(χ^(2)=10.648,P<0.05).Compared the incidence of complications between the two groups,2.34%in the study group was lower than 12.50%in the control group,and the difference was statistically significant(χ^(2)=9.608,P<0.05).There was no statistically significant difference in the SDS scores of the two groups before treatment(t=0.514,P>0.05);after treatment,the SDS points of the study group was lower than the control group,the difference was statistically significant(t=77.450,P<0.05).The operation time and postoperative diet time between the study group and the control group were compared,the difference was statistically significant(t=37.300,34.013,P<0.05).Conclusion Endoscopic mucosal resection is effective in the treatment of colorectal broad-based polyps,which is conducive to the quality control of patients'own treatment.The treatment guidance for patients has high clinical value and can be extended to clinical practice.
