上颌窦内提升同期短种植体植入在严重骨萎缩老年患者的临床效果评价

摘　　要：目的:探究上后牙缺失伴骨高度严重不足的老年患者,经牙槽嵴顶入路行上颌窦底内提升(OSFE),植入与不植入Bio-oss(s)骨粉同期植入短种植体的临床效果。方法:年龄≥60,初始窦嵴距(RBH)≤4mm的患者30例,将8mm种植体随机植入OSFE并植骨(对照组)和不植骨(实验组)的缺牙区,6个月愈合周期后完成修复。比较两组种植体2年存活率及术后1年、2年窦底新生骨高度(ESBG)、种植体周围骨高度和颈部边缘骨吸收情况。结果:本研究共植入32枚种植体(实验组:15枚,对照组:17枚),平均初始RBH为2.5±0.7mm,2年观察期内种植体存活率分别为实验组93.3%和对照组94.1%(P<0.01)。2年后所有存活种植体均获得窦底新生骨(实验组:3.65±1.0mm,对照组:4.89±1.06 mm;P<0.01)。两组的平均骨高度在2年间基本保持稳定。结论:通过2年临床观察,老年人严重骨萎缩的后牙区行上颌窦底内提升植骨或不植骨联合同期植入8mm短种植体可获得理想临床效果。Objective:To investigate the efficacy of short implants placed with OSFE and to compare bone levels around implants placed with(control)or without(test)grafting after 2 years in the elderly patients with severe bone atrophy.Methods:30 patients with age≥60 and initial sinus ridge distance(RBH)≤4mm were randomly implanted with 8mm OSFE implants with bone grafting(control group)or no bone grafting(experimental group).The restoration was completed after a 6-month healing cycle.Peri-implant bone levels were measured on standardized periapical radiographs.Results:Thirty-two implants(15 test,17 control)were placed at a mean RBH of 2.5±0.7mm.The 2-year survival rate was 93.3%and 94.1%for the test and control groups(P<0.01).After 2 years,all implants had gained endo-sinus bone(test:3.65±1.0mm;control:4.89±1.06mm;P<0.01).Mean bone gain was stable in 2 years in both groups.Conclusions:Atrophic posterior maxillae can be achieved the desired outcomes using OSFE and simultaneous placement of 8-mm implants during the 2 years study period in elderly patients.
