糖化血红蛋白水平对糖尿病伴颌面间隙感染患者的预后影响

摘　　要：目的 探讨糖化血红蛋白HbA1c水平对糖尿病伴颌面间隙感染患者的预后影响,为临床治疗颌面间隙感染提供依据。方法 颌面间隙感染并糖尿病患者79例,其中男45例,女34例。据HbA1c值大小分为2组,组ⅠHbA1c小于或等于7.0%,组ⅡHbA1c高于7.0%。分析2组患者平均住院日、白细胞计数、超敏C反应蛋白、并发症及细菌培养等情况。结果 2组间性别和年龄无显著性差异,而2组间疗程、感染范围及抗生素使用强度及治疗转归差异具有显著性。结论 糖尿病患者不同HbA1c水平影响患者颌面间隙感染的治疗预后,肺炎克雷伯菌多见于组Ⅱ患者,控制血糖可提升口腔颌面筋膜感染治愈转归率。Objective To discuss prognostic effect of glycosylated haemoglobin(hemoglobin A1c,HbA1c)level on patients with diabetes mellitus and maxillofacial infection,so as to provide basis for clinical treatment of maxillofacial infection.Methods There were 79 patients with maxillofacial interstitial infection and diabetes mellitus.Among them,45 were male and 34 were female.It divided them into two groups according to HbA1c value,HbA1c of groupⅠwas less or equal to 7.0%,and HbA1c of group was higher than 7.0%.The average hospital stay,white blood cell count,ultrasensitive C-reactive protein,complications and bacterial culture and so on were analyzed in both groups.Results There was no significant differences in gender and age between the two groups,while there was significant differences in duration of treatment,extent of infection and intensity of antibiotic use and treatment regression between the two groups.Conclusions Different HbA1c levels in diabetic patients affect the prognosis of treatment of maxillofacial interstitial infections in patients,Klebsiella pneumoniae is common in groupⅡpatients,and glycaemic control enhances the rate of healing regression of oral and maxillofacial infections.
