摘  要:目的:研究粪便隐血与组织因子途径抑制物-2(tissue factor pathway inhibitor-2,TFPI-2)基因甲基化联合检测在结直肠癌中的应用价值。方法:将我院收治的100例结直肠癌患者作为观察组,另选同期健康体检志愿者48名作为对照组,收集2组人员的粪便作为检测标本,检测2组人员粪便标本TFPI-2基因甲基化和粪便隐血结果,分析两者联合检测对结直肠癌的应用价值。结果:观察组患者粪便隐血阳性率为29.00%,对照组为0,差异有统计学意义(P <0.05);观察组患者TFPI-2基因甲基化阳性率64.00%,对照组为0,差异有统计学意义(P <0.05);观察组患者TFPI-2基因甲基化阳性率为64.00%,高于粪便隐血阳性率29.00%,差异有统计学意义(P <0.05)。粪便隐血和TFPI-2基因甲基化单独检测诊断AUC为(0.718)和(0.801),低于联合检测诊断AUC(0.903),根据ROC曲线获得,最佳界限粪便隐血为(86.7)、TFPI-2基因甲基化为(94.0),联合检测为(97.6),可获得ROC曲线下最大面积,联合检测敏感度高于单独检测。结论:粪便隐血和TFPI-2基因甲基化联合检测对结直肠癌早期筛查比单独检测具有更高的效率和更好的效果,能够为结直肠癌早期筛查起到循证作用。Objective To study the value of combined detection of fecal occult blood and tissue factor pathway inhibitor-2(TFPI-2)gene methylation in colorectal cancer.Methods 100patients with colorectal cancer treated in author’s hospital were selected as the observation group and 48healthy volunteers in the same period were selected as the control group.The feces of the two groups were collected as test samples.The results of TFPI-2gene methylation and fecal occult blood in the feces of the two groups were detected,and the application value of the combined detection of the two groups for colorectal cancer was analyzed.Results There were all significant difference between the two groups in the positive rate of fecal occult blood(29.00% vs 0,P <0.05)and in the positive rate of TFPI-2gene methylation(64.00%vs 0,P<0.05);The positive rate of TFPI-2gene methylation was higher than that of fecal occult blood in the observation group(64.00% vs 29.00%,P <0.05).The AUC of fecal occult blood and TFPI-2gene methylation detected separately is(0.718)and(0.801),which is lower than that of combined detection(0.903).According to the ROC curve,the best limit of fecal occult blood is(86.7),TFPI-2gene methylation is(94.0),and the combined detection is(97.6).The maximum area under the ROC curve can be obtained,and the sensitivity of combined detection is higher than that of single detection.Conclusion The combined detection of fecal occult blood and TFPI-2gene methylation has higher efficiency and better effect on early colorectal cancer screening than single detection,and can play an evidence-based role in early colorectal cancer screening.
