术中高血压对胃癌根治术后患者总生存期的影响

摘　　要：目的观察术中高血压对胃癌根治术后患者总生存期的影响。方法选取2014年1月至2019年4月在郑州大学第一附属医院接受胃癌根治术的1269例胃癌患者,根据术中有无出现高血压分成两组,高血压组289例[收缩压(SBP)≥160mmHg,1mmHg=0.133kPa],非高血压组980例(SBP<160mmHg)。随访比较两组患者术后总生存期。结果单因素分析结果显示,术中SBP≥160mmHg组与SBP<160mmHg组相比,1a生存率分别为89.60%、91.90%,风险比为1.300(95%CI0.988~1.710,P=0.060),差异无统计学意义;多因素分析显示,术中血压SBP≥160mmHg组相比较于SBP<160mmHg组对胃癌术后总生存期的影响无统计学意义(P>0.05)。结论术中高血压对胃癌根治术后患者总生存期无显著影响。Objective To observe the effect of intraoperative hypertension on the overall survival after radical gastrectomy for gastric cancer.Methods A total of 1269 patients who underwent radical gastrectomy for gastric cancer in the First Affiliated Hospital of Zhengzhou University from January 2014 to April 2019 were selected.Patients were divided into two groups based on the presence or absence of hypertension.There were 289 patients in hypertensive group[systolic blood pressure(SBP)≥160 mmHg,1 mmHg=0.133 kPa]and 980 patients in non-hypertensive group(SBP<160 mmHg).The two groups were followed up to compare the difference in postoperative overall survival.Results Univariate analysis showed that the 1-year survival rate of SBP≥160 mmHg group and SBP<160 mmHg group was 89.60%and 91.90%,respectively,and the hazard ratio was 1.300(95%CI 0.988-1.710,P=0.060),and the difference was not statistical.In multivariate analysis,there was no statistical difference in the influence of intraoperative blood pressure SBP≥160 mmHg and SBP<160 mmHg on postoperative survival(P>0.05).Conclusion Intraoperative hypertension has no significant effect on postoperative overall survival of patients undergoing radical gastrectomy.
