系统性语言护理在脑卒中运动性失语患者中的应用价值

摘　　要：目的 探究系统性语言护理在脑卒中运动性失语患者中的应用效果。方法 在本院神经内科2018年4月至2019年10月诊治的脑卒中患者中选出127例运动性失语患者,随机将患者分为两组:对照组患者给予常规护理措施,观察组在此基础上给予系统性语言护理,比较两组患者的语言功能评分、负性情绪、神经功能缺损程度、脑血流指标、生活质量评分。结果 观察组护理3个月后语言功能的口语理解、自发谈话、复述能力和命名能力评分均高于对照组,P <0.05;且两组护理3个月后的SAS焦虑评分、SDS抑郁评分均有所下降,且观察组较对照组下降更明显,P <0.05;观察组护理3个月后的NIHSS评分低于对照组,P <0.05;且观察组护理3个月后的左大脑中动脉血流速度高于对照组,P <0.05,而两组左大脑前动脉、后动脉的血流速度差异无统计学意义,P> 0.05;观察组护理3个月后生活质量(SAQOL-39g)的生理、沟通、心理社会评分均高于对照组,P <0.05;观察组患者对护理服务的满意度96.88%高于对照组的护理满意度85.71%,P <0.05。结论 系统性语言护理为脑卒中运动性失语患者开展有针对性的、循序渐进的语言训练,逐步恢复患者的语言功能,消除负性情绪,促进患者神经功能修复,提高生活质量。Objective To explore the effect of systematic language nursing in patients with motor aphasia after stroke.Methods One hundred and twenty seven patients with stroke were selected from April 2018 to October 2019 in the department of neurology.All patients were randomly divided into two groups:the control group was given routine nursing measures,the observation group was given systematic language nursing,and the language function scores of the two groups were collected.Results The scores of oral comprehension,spontaneous conversation,retelling and naming ability of the observation group were higher than those of the control group after 3 months of nursing,P<0.05.And the scores of SAS anxiety after 3 months of nursing the scores of SDS depression in the observation group were lower than those in the control group,P<0.05.The NIHSS score of the observation group after 3 months of nursing was lower than that of the control group,P<0.05.The blood flow velocity of left middle cerebral artery in the observation group after 3 months of nursing was higher than that in the control group,P<0.05.There was no significant difference in blood flow velocity between the two groups,P>0.05.The physiological,communication and psychosocial scores of quality of life(SAQOL-39g) in the observation group after 3 months of nursing were higher than those in the control group,P<0.05.And the satisfaction of the patients in the observation group was 96.88% higher than that in the control group,85.71%,P<0.05.Conclusion Systematic language nursing for stroke motor aphasia patients to carry out targeted,gradual language training,gradually restore the language function of patients,eliminate negative emotions,promote the repair of their neurological function and improve their quality of life.
