摘　　要：目的探讨ProTaper机用镍钛锉在儿童乳磨牙根管治疗中的应用效果及对填充效果及根管清洁情况的影响。方法选取2020年4月至2021年4月于本院口腔科接受乳磨牙根管治疗的180例患儿作为研究对象,以随机数字法将其分为对照组和观察组,每组90例(90颗,270个根管)。对照组采用手用不锈钢K锉治疗,观察组采用ProTaper机用镍钛锉治疗。比较两组患儿的治疗效果。结果观察组的单根根管预备时间、根管填充时间短于对照组,临床治疗成功率高于对照组,差异具有统计学意义(P<0.05)。观察组的根管预备形态良好率高于对照组,填充效率优于对照组,差异具有统计学意义(P<0.05)。术后24 h,两组患儿的龈沟液IL-1β、IL-6及TNF-α水平均降低,且观察组显著低于对照组,差异具有统计学意义(P<0.05)。术后24 h,两组患儿的FLACC评分均降低,且观察组显著低于对照组,差异具有统计学意义(P<0.05)。观察组的碎屑评分与玷污层评分显著低于对照组,差异具有统计学意义(P<0.05)。结论ProTaper机用镍钛锉应用于儿童乳磨牙根管治疗中,可明显缩短根管预备与填充时间,有效提高患儿乳磨牙填充效果,改善根管预备形态、牙龈炎症反应,降低患儿疼痛感,且能有效清洁根管,值得推广。Objective To investigate the application effect of ProTaper nickel-titanium file in root canal therapy of deciduous molar in children and its influences on fill effect and root canal cleaning.Methods A total of 180 children received root canal therapy of deciduous molar in the stomatology department of our hospital from April 2020 to April 2021 were selected as the research objects.The children were divided into control group and observation group by random number method,with 90 cases(90 teeth,270 root canals)in each group.The control group was treated with stainless steel hand K file,and the observation group was treated with ProTaper nickel-titanium file.The therapeutic effects of the two groups were compared.Results The preparation time of single root canal and root canal fill time in the observation group were shorter than those in the control group,the success rate of clinical treatment was higher than that in the control group,and the differences were statistically significant(P<0.05).The good rate of root canal preparation shape in the observation group was higher than that in the control group,fill effect was better than that in the control group,and the differences were statistically significant(P<0.05).At 24 h after operation,the levels of IL-1β,IL-6 and TNF-αof gingival crevicular fluid in both groups decreased,those in the observation group were significantly lower than the control group,and the differences were statistically significant(P<0.05).At 24 h after operation,the FLACC score in both groups decreased,that in the observation group was significantly lower than the control group,and the difference was statistically significant(P<0.05).The detritus score and smear layer score in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion ProTaper nickel-titanium file applied in root canal therapy of deciduous molar in children can significantly shorten the time of root canal preparation and filling,effectiv
