品管圈在DIP下病案首页质控中的应用

摘　　要：目的探究品管圈(QCC)在病种分值付费(DIP)下病案首页质量控制管理中的应用。方法将2021年1—4月未实施QCC活动前基于DIP的病案首页12654份作为对照组,2021年5—12月实施QCC活动后基于DIP的病案首页26354份作为QCC组。对照组病案行常规病案首页质控,QCC组利用QCC质量管理工具开展QCC改善活动。分析比较两组病案首页信息合格率、病案首页填写缺陷情况、病案首页手术信息提取率、圈员责任评分(积极性、品管手法、团队精神、沟通协调能力、解决问题能力)。结果QCC组病案合格率显著高于对照组,差异有统计学意义(P<0.05)。QCC组病案首页整体缺陷率为4.01%,对照组为7.72%,两组整体缺陷比较,差异有统计学意义(P<0.05)。QCC组由病案首页提取开放手术、微创手术、四级手术信息提取率显著高于对照组,差异有统计学意义(P<0.05)。QCC后圈员积极性、品管手法、团队精神、沟通协调能力、解决问题能力评分均较QCC前有显著升高,差异有统计学意义(P<0.05)。结论QCC可显著改善DIP下病案首页合格率,降低病案首页填写缺陷率,提高由病案首页提取手术信息概率,改善圈员工作态度。Objective To explore the application of quality control circle(QCC)in the quality control management of medical record homepage under disease-specific payment(DIP).Methods 12654 front pages of DIP-based medical records before the implementation of QCC activities from January to April 2021 were used as the control group.After the implementation of QCC activities from May to December 2021,26354 front pages of DIP-based medical records will be used as the QCC group.The medical records of the control group were subject to routine medical record homepage quality control,and the QCC group used QCC quality management tools to carry out QCC improvement activities.Analyze the comparison of the information qualification rate of the two groups of medical cases,the defects of the homepage of the medical case,the information extraction rate of the surgical information of the medical case,and the responsibility score of the staff(enthusiasm,quality management technique,team spirit,communication and coordination ability,and problem solving ability).Results The qualified rate of medical records in the QCC group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).The overall defect rate of the first page of medical records in the QCC group was 4.01%,and that in the control group was 7.72%,there was a statistically significant difference between the two groups(P<0.05).The extraction rate of open surgery,minimally invasive surgery,and fourth-level surgery information extracted from the first page of medical records in the QCC group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).The scores of enthusiasm,quality control techniques,team spirit,communication and coordination ability,and problem-solving ability of the circle members after QCC were significantly higher than those before QCC, and the difference was statistically significant(P<0.05).
