摘 要:目的:探讨钛种植体愈合期牙龈炎性浸润及巨噬细胞极化。方法:15只SD大鼠上颌磨牙区植入埋入式愈合的种植体作为实验组,右侧切开黏骨膜翻瓣后缝合作同体对照。5只未作处理的大鼠作为空白对照组,愈合后1、4、6周取创口部位牙龈,HE染色观察组织学特点;免疫荧光观察巨噬细胞浸润特征及极化特点;qPCR分析细胞因子表达。结果:种植体周围牙龈组织愈合过程呈现更高密度的炎细胞浸润,其中巨噬细胞呈M2极化趋势。qPCR结果显示种植体周围软组织内细胞因子表达以抗炎促修复为主。结论:钛种植体可增强牙龈组织内免疫活动,影响巨噬细胞极化产生利于组织修复的免疫微环境,提示牙龈内发生一定程度的异物反应。Objective:To study gingival inflammatory infiltration and macrophage polarization during titanium implant healing.Methods:The implants were implanted at one side in the maxillary molar area of 15 SD rats as the experimental group,and the mucoperiosteal flap was cut and sutured on another side as the control group.5 untreated rats were used as the blank controls.The gingiva of the wound site was taken at 1,4 and 6 weeks respectively(n=5)after healing,and the histological characteristics were observed by HE staining,the characteristics of macrophage infiltration and polarization were observed by immunofluorescence,and the expression of inflammatory cytokines was analyzed by qPCR.Results:The healing process of gingival tissue around the implant showed a higher density of inflammatory cell infiltration,in which macrophages showed the trend of M2 polarization.qPCR showed that the expression of inflammatory cytokines in the soft tissue around the implant was mainly anti-inflammatory and promoting repair.Conclusion:Titanium implant can enhance the immune activity in gingival tissue and affect the polarization of macrophages to produce an immune microenvironment conducive to tissue repair,suggesting that there is foreign body reaction in the gingiva.
