摘 要:目的:探讨非痴呆血管性认知障碍(VCI-ND)中命名功能和其他认知功能之间的关系。方法:比较56例VCI-ND患者和50名健康志愿者在整体认知功能(MMSE)、记忆(AVLT)、执行/注意(STT、SCWT、RME、DOT-A、SDMT)、语言(ANT、BNT、FFT、ST、VFT)和视觉空间功能(CFT)方面的表现。使用ROC曲线筛选适合评估VCI-ND患者语言功能的最佳测试。分析该语言测试与其他认知功能的相关性。根据该测试将VCIND患者分为伴或不伴命名功能损害的VCI-ND两组,并观察其认知特点。结果:病例组的命名功能显著差于对照组(P<0.05,P<0.01)。最适合评估VCI-ND的语言测试是BNT(曲线下面积为0.75)。BNT与DOT-A(r=0.28)、RME(r=0.48)、STT(r=-0.39)和CFT(r=0.49)存在相关性(P<0.05)。伴命名功能损害的VCI-ND患者在语言测试中的ANT[(28.41±3.99)vs(32.17±1.19)]和ST[(12.50±5.82)vs(17.21±3.50)]以及执行测试中的STT-B[(305.52±107.63)vs(238.63±61.86)]表现显著差于不伴命名功能损害的VCI-ND患者(P<0.05,P<0.01)。结论:BNT测试检测VCI-ND患者的命名功能优于其他语言测试,并与视空间能力和执行功能相关。VCI-ND患者的命名功能与执行功能表现密切相关。Objective:To investigate the relationship in vascular cognitive impairment-no dementia(VCI-ND)between language function and other cognitive functions.Methods:The performance on the cognitive screening(MMSE),memory(AVLT),executive function/attention(STT,SCWT,RME,DOT-A,SDMT),language(ANT,BNT,FFT,ST,VFT),and visuospatial function(CFT)between 56 VCI-ND patients and 50 healthy volunteers was compared.Then,the receiver operating characteristic(ROC)curve was used to find the best test to assess language function in patients with VCI-ND.The correlation between the language test and other cognitive functions was analyzed.VCI-ND patients were divided into two groups according to language impairment and observed their characteristics.Results:Individuals with VCI-ND performed significantly worse than those in the control group in terms of language(P<0.05,P<0.01).The language test with the largest predictive power with respect to VCI-ND was the BNT(AUC=0.75).BNT was correlated with DOT-A(r=0.28),RME(r=0.48),STT(r=-0.39)and CFT(r=0.49)(P<0.05).VCI-ND patients with language dysfunction were significantly worse than VCI-ND patients without language dysfunction in the performance of ANT[(28.41±3.99)vs(32.17±1.19)]and ST[(12.50±5.82)vs(17.21±3.50)]in language test and STT-B[(305.52±107.63)vs(238.63±61.86)]in executive test(P<0.05,P<0.01).Conclusion:BNT test is suitable for language function detection in VCI-ND,related to visuospatial and executive function.The naming function of VCI-ND patients is closely related to executive function.
