摘　　要：目的 探讨垂体泌乳素瘤患者血清PRL、Cor和胰岛素抵抗状况变化及临床意义。方法 选择98例垂体泌乳素瘤患者作为研究对象,所有患者在术前、术后7 d清晨采集空腹肘静脉血测定其血清PRL、Cor水平,计算胰岛素抵抗指数(HOMA-IR)。比较微腺瘤、大腺瘤患者和复发、未复发患者在术前、术后7 d的血清PRL、Cor和HOMA-IR水平。结果 大腺瘤患者在术前、术后7 d的血清PRL、Cor和HOMA-IR水平均明显高于微腺瘤患者,两组在术后7 d的血清PRL、Cor和HOMA-IR水平均明显低于术前,差异有统计学意义(P<0.05)。复发患者和未复发患者在术前的血清PRL、Cor和HOMA-IR水平比较无统计学意义(P>0.05),两组在术后7 d的血清PRL、Cor和HOMA-IR水平均明显低于术前,复发患者在术后7 d的血清PRL、Cor和HOMA-IR水平均明显高于未复发患者,差异有统计学意义(P<0.05)。结论 垂体泌乳素瘤患者的血清PRL、Cor和HOMA-IR水平变化能够在一定程度上体现其肿瘤体积,对于评价其治疗效果、预测术后复发具有一定的临床意义。Objective To research the changes and clinical significance of serum PRL,Cor and insulin resistance in patients with pituitary prolactinoma.Methods 98 patients with pituitary prolactinoma were selected as the research objects.Fasting elbow venous blood was collected from all patients before operation and 7 days after operation to determine their serum PRL and Cor,and calculated homeostasis model assessment of insulin resistance(HOMA-IR).The serum levels of PRL,Cor and HOMA-IR in patients with microadenoma,macroadenoma and recurrence,non-recurrence were compared before and 7 days after operation.Results The levels of serum PRL,Cor and HOMA-IR of macroadenoma patients before and 7 days after operation were significantly higher than those of microadenoma patients,the levels of serum PRL,Cor and HOMA-IR of the 2 groups on 7 days after operation were significantly lower than those before operation(P<0.05).There were no significant differences in serum PRL,Cor and HOMA-IR between the 2 groups before operation(P>0.05).The levels of serum PRL,Cor and HOMA-IR the 2 groups were significantly lower than those before operation on the 7 th day after operation,and the serum PRL,Cor and HOMA-IR the recurrent patients were significantly higher than those of the non-recurrent patients on the 7 th day after operation(P<0.05).Conclusion The changes of serum PRL,Cor and HOMA-IR in patients with pituitary prolactinoma can reflect the tumor volume to a certain extent,which has certain clinical significance for evaluating the therapeutic effect and predicting postoperative recurrence.
