脊柱骨巨细胞瘤手术治疗2年内复发情况及相关危险因素分析

摘　　要：目的 分析脊柱骨巨细胞瘤手术治疗2年内复发情况及相关危险因素。方法 选取行脊柱骨巨细胞瘤手术患者65例,收集患者性别、年龄、肿瘤最大直径、患病节段、胰岛素样生长因子ⅡmRNA结合蛋白3(IMP3)表达、手术方式、侵袭范围等信息,统计术后2年复发率,分析复发的危险因素。结果 65例患者均获得有效随访,2年内复发15例,复发率23.08%(15/65)。患病节段、手术方式、IMP3表达、侵袭范围和是否使用双膦酸盐、术后放疗是脊柱骨巨细胞瘤手术治疗2年内复发的影响因素(P<0.05);logistic回归分析显示,患病节段腰椎、次全椎节切除术、IMP3表达阳性、侵袭范围室间外、未使用双膦酸盐、术后未放疗是脊柱骨巨细胞瘤手术治疗2年内复发的危险因素(P<0.05)。结论 脊柱骨巨细胞瘤手术后复发率高,患病节段腰椎、次全椎节切除术、IMP3表达阳性、侵袭范围室间外、未使用双膦酸盐、术后未放疗均会增加复发风险,临床应据此实施针对性干预措施,以降低复发率。Objective To analyze the recurrence and risk factors of giant cell tumor of spinal bone in 2 years.Methods 65 patients were selected,to collect information on the patient’s sex,age,tumor maximum diameter,diseased segment,insulin-like growth factor ii mRNA binding protein 3(IMP3) expression,surgical approach,invasion range,The recurrence rate was 2 years after operation,analyze the risk factors for recurrence.Results 65 patients received effective follow-up,Recurrence 15 cases within 2 years,The recurrence rate was 23.08%(15/65).The disease segment,operation mode,IMP3 expression,invasion range and whether to use bisphosphonates,postoperative radiotherapy are the influencing factors of recurrence in 2 years of surgical treatment of giant cell tumor of spine bone(P<0.05);logistic regression analysis showed,risk factors for recurrence within 2 years of surgical treatment of diseased segment lumbar vertebrae,subtotal vertebroplasty,positive IMP3 expression,invasive range extraventricular,non-use of bisphosph no postoperative radiotherapy for giant cell tumor of the spine(P<0.05).Conclusion The recurrence rate of giant cell tumor of spine is high after operation,Disease segment lumbar vertebrae,subtotal vertebroplasty,positive IMP3 expression,invasion range,extraventricular,non-use of bisphosphonates,no postoperative radiotherapy,clinical intervention should be carried out accordingly to reduce the recurrence rate.
