摘  要:目的研究非小细胞肺癌(NSCLC)癌组织中第10号染色体同源丢失性磷酸酶张力蛋白基因(PTEN)、Notch受体1(Notch 1)表达情况与其淋巴结转移和预后的相关性。方法将2016年1月至2019年1月河北省胸科医院收治的188例NSCLC患者纳入本次回顾性研究,患者均口服替尼类靶向药物进行治疗,随访3年,根据患者是否死亡将其分为死亡组和生存组。均行肺部活检,取癌组织及癌旁组织,比较癌组织以及癌旁组织PTEN、Notch 1表达;比较不同临床特征、不同预后的患者癌组织PTEN、Notch 1表达;分析预后不良及肿瘤淋巴结转移与癌组织中PTEN、Notch 1表达的相关性。结果NSCLC癌组织的PTEN蛋白阳性表达率为21.28%,较癌旁组织(89.89%)显著降低,而Notch 1蛋白阳性表达率为75.00%,则较癌旁组织(4.26%)显著升高,差异均有统计学意义(P<0.05)。不同性别、年龄患者癌组织中PTEN、Notch 1蛋白阳性表达差异均无统计学意义(P>0.05);TNM III+IV期、腺癌及有淋巴结转移患者癌组织的PTEN蛋白的阳性表达率分别较对应的TNM I+II期、鳞癌及无淋巴结转移患者显著降低,而Notch 1蛋白阳性表达率则显著升高,差异均有统计学意义(P<0.05)。死亡组患者癌组织中的PTEN蛋白阳性表达率为0,则较存活组(33.06%)显著降低,而Notch 1阳性表达率为98.51%,较存活组(61.98%)显著增加,差异均有统计学意义(P<0.05)。Spearman相关性分析显示,NSCLC的淋巴结转移以及不良预后与癌组织中Notch 1呈正相关(r=0.671、0.821,P<0.05),与PTEN蛋白表达呈负相关(r=-0.563、-0.736,P<0.05)。结论NSCLC的淋巴结转移及不良预后与癌组织中Notch 1蛋白表达呈正相关,与PTEN蛋白表达呈负相关,可为临床上NSCLC的诊断、治疗、病情及预后评估提供一定的依据。Objective To investigate the correlation of the expression of gene of phosphate and tension homology deleted on chromosome 10(PTEN)and notch receptor 1(Notch1)with lymph node metastasis and prognosis of non-small cell lung cancer(NSCLC).Methods A total of 188 NSCLC patients admitted to Hebei Chest Hospital from January 2016 to January 2019 were retrospectively included in this retrospective study.All patients were treated with targeted drugs of tinib orally.They were followed up for 3 years and divided into death group and survival group according to whether they died.The expression of PTEN and Notch1 in cancer tissues and adjacent tissues were compared.The expression of PTEN and Notch1 in cancer tissues of patients with different clinical characteristics and prognosis was compared.To analyze the correlation between poor prognosis and lymph node metastasis with the expression of PTEN and Notch1 in cancer tissues.Results The positive expression rate of PTEN protein in NSCLC cancer tissues was 21.28%,which was significantly lower than that in adjacent tissues(89.89%),while the positive expression rate of Notch 1 protein was 75.00%,which was significantly higher than that in adjacent tissues(4.26%),the differences were statistically significant(P<0.05).There were no significant differences in the positive expression of PTEN and Notch1 protein in cancer tissues of different genders and ages(P>0.05).The positive expression rate of PTEN protein in cancer tissues of patients with TNM stage III+IV,adenocarcinoma and lymph node metastasis was significantly lower than that of the corresponding TNM stage I+II,squamous cell carcinoma and no lymph node metastasis,while the positive expression rate of Notch 1 protein was significantly higher,the differences were statistically significant(P<0.05).The positive expression of PTEN protein in the cancer tissue of the death group was 0,which was significantly lower than that of the survival group(33.06%),while the positive expression of Notch 1 was 98.51%,which was significantly hig
