摘  要:胃食管反流性咳嗽(GERC)是慢性咳嗽的常见病因之一,近年来发现GERC的比例较高,长期使用抑酸剂会造成一定的不良反应且对难治性GERC的效果较差,反复发作的咳嗽严重影响患者的生活质量。GERC发病机制尚未完全明确,感觉神经贯穿GERC的多个环节。外周与中枢神经敏化之间相互作用,可能放大了炎症效应,触发机体的咳嗽冲动。瞬时受体电位离子通道作为感觉神经纤维的重要通道蛋白,参与、介导神经纤维信号的形成与传导,能够加强外周与中枢神经之间的联系,可能是GERC的重要发病机制。因此,基于外周与中枢神经敏化对GERC发病机制的进一步探究可能为其治疗提供新思路。Gastroesophageal reflux cough(GERC)is one of the common causes of chronic cough,which has been found to be increasing in chronic cough.Long-term use of acid suppressors has obvious adverse effects,and the effect on refractory GERC is not obvious.The recurrent cough seriously affects the quality of life of the patients.The specific pathogenesis of GERC has not been fully clarified,while it′s known that sensory nervous system plays a role throughout the pathogenesis of GERC.Sensitization of peripheral nerves and central nervous system interacts with each other,which may amplify the peripheral inflammatory effects and trigger cough.As an important channel protein of sensory nerve fibers,transient receptor potential ion channel participates in and mediates the formation and transmission of nerve fiber signals,and can strengthen the connection between peripheral and central nerves,which may be an important pathogenesis of GERC.Therefore,further research on the pathogenesis of GERC based on peripheral and central nervous sensitization may provide new ideas for the treatment.
